Job Description

Support Planner, Sherwood Street Centre (move on support in dispersed housing), Mansfield, Permanent, FRWK06 £18,476 - £20,531, 37 hpw

Sherwood Street Centre is a service that provides 15 units of core accommodation and 39 units of dispersed housing in the community.

We are looking for reliable and self-motivated candidates who have experience of working in a community setting supporting individuals in tenancies managed by Framework.

You will be expected to prepare and document support plans including managing risk, need, engagement, and to help service users achieve objectives and move forward to more permanent accommodation.

A knowledge of tenancy issues would also be useful and you will be expected to work alongside housing management to address any tenancy related issues / concerns.

There may be occasions when you will be required to support the core hostel, so an understanding of issues faced by people affected by homelessness, and an awareness of support needs of clients who have complex needs is essential

The main functions of this role will be:

· To build the skills of service users to empower them to maximise their independence

· To contribute to the achievement of contractual obligations

· To work in partnership with other agencies to maximise service delivery

· To assess individual service users skills and needs and to work positively with risk

· To prepare documented assessments and support plans with service users

· To support the service users to engage with move on plans

Due to the demographics of the properties access to the car for work purposes is necessary.
