The Targeted Support Service is a service that provides Floating support to tenants of Frameworks Housing Management service.
Properties are located across Nottinghamshire as well as the City so a car for work purposes and a full driving licence is required
In addition to the Floating support service we have a team member working with the Private Rented Sector to provide opportunities for appropriate move on accommodation.
Both Services are based at Val Roberts House in Nottingham.
The service is made up of a Team Leader, 3 x Support Planners, 3 x Support Workers and 1x Private Rented Accommodation Planner.
A Planner is responsible for Support Planning for up to 33 allocated number of tenants in partnership with one allocated Support Worker, Primary Reviews are carried out every six months and interim reviews every 3 months are also carried out with the support worker who delivers support, planners are required to assist and work with Tenants at times also.
We are a small busy team so the ability to be a team player with excellent time management skills is essential.
Targeted Support aim is to provide support to Framework tenants across 97 units of accommodation so therefore caseload changes occur as Tenants move out and new tenants move in the units.
We complete all assessments based on this movement and do not take any outside referrals.
We work very closely with the Housing Management team, so an ability to foster good working and positive relationships with another team is very important as is an understanding of their role as well as your own.
The ability to complete concise and clear support plans in electronic form is a must, as is a good working knowledge of inform and the move-on pathways into independent accommodation options available as move-on is a large part of this role.
