Job description

SUPPORT PLANNER, SHERWOOD STREET CENTRE, MANSFIELD, FRWK06 £19,334 - £21,415, PERMANENT, 37 HPW

Sherwood Street Centre is a 24-hour service that provides 15 units of core accommodation for 15 individuals with complex needs and also 39 units of dispersed housing in the community. 

This vacancy is for 24 – hour accommodation.

You will be expected to prepare and document support plans including managing risk, need, engagement, and to help service users achieve objectives and move forward to more permanent accommodation.

To have an understanding of issues faced by people affected by homelessness, and an awareness of support needs of clients who have complex needs is essential 

The main functions of this role will be:

• To build the skills of service users to empower them to maximise their independence 

• To contribute to the achievement of contractual obligations 

• To work in partnership with other agencies to maximise service delivery 

• To assess individual service users skills and needs and to work positively with risk 

• To prepare documented assessments and support plans with service users 

• To support the service users to engage with move on plans 

Work pattern will include evenings, weekends & bank holidays.
