Job description

SUPPORT PLANNER, HOUSING RELATED SUPPORT NORTH EAST LINCOLNSHIRE, PERMANENT, 37 HPW, FRWK06 £20,127 - £22,293

HRS North East Lincolnshire is a community based support service working in Grimsby, Cleethorpes and the surrounding areas.  It provides housing related support to people who are assessed at risk of losing their current accommodation or are currently homeless and need support to find a place to live.

We are looking for candidates who can communicate with people in a friendly and welcoming manner. You will need to be reliable and enthusiastic with an understanding of safeguarding. Experience of working in a similar service is not necessary but preferable.

The Support Planner role provides support to service users in their own home or in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs in order to prevent homelessness and secure and sustain accommodation.

The main functions of the role are;

  • To risk assess all referrals and develop appropriate risk management strategies enabling service users to take responsibility and address challenges.
  • To prepare agreed support plans with Service Users, regularly review and update aforesaid plans
  • To ensure that tasks identified tasks are clear, appropriate and agreed with the service user
  • To liaise appropriately with key partners, stakeholders and service providers to ensure appropriate support is offered to all service users.
  • Carry out a client risk assessment with every new Service User identifying with them the risks presented by the service user and the environment.
  • Complete agreed Risk Management Plans/ Strategies ensuring these enable service users to take responsibility and address challenges.
  • Ensure Risk Management strategies are clear, appropriate and relevant and are communicated to the rest of the management/support team.
  • Regularly review risk assessments and risk management plans/strategies.

The role will require you to work flexibly to adapt to the needs of the service

You must have a current driving licence and access to a car in order to cover North East Lincolnshire.
