Job Description

SUPPORT WORKER, MOVING FORWARD SOUTH NOTT’S (COVERING RUSHCLIFFE, GEDLING AND BROXTOWE DISTRICTS), PERMANENT, FRWK03 £16,296 - £17,811 FTE, 37HPW

Moving Forward is a community based support service for people with mental health problems based at various locations within Nottinghamshire. We work in partnership with the Community Mental Health Teams to provide support to people to live independently in their own home.

In this role you will deliver individually planned support to people to help them achieve their goals, and help them review their progress and achievements. You will be expected to work effectively within a small team, carry your own caseload and support your colleagues. You will be expected to have a good working knowledge of the needs of people struggling with their mental health, such as benefits, housing law, debt support and any other issues related to the person’s ability to live independently. You will have experience of writing up and recording support notes and good communication skills.  You must have a current driving licence and access to a car in order to cover multiple bases across various districts in the County.
