Job description

The H4 Hospital Project provides discharge support, housing and health advocacy, and ongoing wellbeing support to homeless patients. It is run by the charity H3 which supports people experiencing homelessness within Stockport to build independence and wellbeing.

The key function of this role is to work alongside the Homeless Health Advocate, housing and hospital staff and others with responsibility for the discharge of patients to provide safe and supported discharge for homeless patients. The Homeless Hospital Advocate will provide outreach services to homeless people and carry out housing assessments on the wards.

The worker will be involved from patient admission or referral through to discharge and placement into adequate accommodation. The initial focus will be to explore all avenues of accommodation, facilitating referrals and meetings to ensure that homeless patients are discharged with suitable accommodation.

The worker will need to build strong working relationships with partner agencies in and out of hospital that can provide support for the service users being advocated for. The worker will represent the wider work and vision of the H4 Hospital Project in the community.

This role provides practical, flexible housing options advocacy and work is carried out across Stockport and the surrounding boroughs in Greater Manchester.
