Job description

Hertfordshire Mind Network are looking for Community Outreach Workers to join our Community Support Service.

Community Outreach Worker
Responsible to: Community Support Service Team Leader
Based: East and West Hertfordshire
Working hours: Full and part time roles
Rate of pay: £22,500 – £23,500 per annum pro rata

You will be responsible for supporting people with complex needs who are suffering with mental ill health. This includes drug and alcohol difficulties. Particularly when they are coming into repeat contact with Police, Tenancy Sustainment Services, Housing and Homelessness Services and Environmental Health Services.

Working within our community support provision you will provide advice, information, onward referral and holistic support to clients with housing issues and coexisting drug and alcohol difficulties and mental ill-health.

Being able to drive and owning your own vehicle is essential for this role.
