Job description
This post progresses equipment grant and emergency loan applications that require an urgent response to support children with disabilities, and their immediate families. This is done through the review of application information from the family and their local supporting professional and interviewing them to ensure a holistic understanding of their need for community equipment.

These applications will be of an urgent nature, you must have the ability to work at a pace and prioritise workload while supporting families in crisis and managing family expectations around their ability to resolve the crisis.

Crisis Support Nurse Responsibilities:

• Conduct/manage casework of urgent grant applications, through assessments to inform decisions associated to the provision of equipment through the commitment of funds.
• Capture and record information accurately to make informed decisions on the need for urgent grants through a matrix of considerations and identify associated risks to the child and family’s safety, health and wellbeing.
• Conduct/manage casework of emergency loan applications to address immediate needs where there is a possible delay to long-term provision or a clear responsibility for involvement by statutory services and where a child is end of life/ deteriorating and their family requires a humanitarian response.
• Liaise with local professionals involved in providing direct care to the child/young person and their family through completing interviews to gather additional information needed to progress an application through to authorisation of funding.
• Raise safeguarding concerns to their Safeguarding Lead.
• Contribute to the continuous improvement of people, systems and processes.

Crisis Support Nurse Requirements:

• Ideally you will have NMC registration, associated to paediatric care and experience in service delivery to disabled children
• You will have an understanding and experience of health and/or safety risk assessment based on information given by Families/Professionals to make decisions.
• Be able to draw on knowledge and skills of workers from other professions and work in collaboration to promote family engagement with statutory services.
• Have a strong understanding Safeguarding processes/pathways.
• You will be able to demonstrate a professional and empathic communication style.
• Be able to prioritise workload.
• Have a good working knowledge of databases and Microsoft packages.
