Help build a bridge!

When a child struggles with their mental health, their parents can seek help from their GP but there is often a wait for further specialist services. You can help bridge that gap.

As a Care Coordinator specialising in children’s mental health and based in GP practices in Sheerness, you will support children and families to access other community or social activities that offer opportunities for the child to develop confidence, self-esteem and resilience. Alternatively, those services may provide support to the parents or family as a whole.

Your time and expertise will support the child and family to understand conversations with primary health professionals, enabling them to make informed choices about their own health and wellbeing through the development of a personalised plan of care. You should be able to quickly connect and build trust with children, young people and families.

This Care Coordinator role is a great opening into Social Prescribing, a growing field in health and social care. We are looking for an organised, self-motivated and confident individual to join our established team. Applicants must have experience of working with children, a background in education, medical or social care, and of supporting individuals to access wider services. Car driver essential.

You will work alongside Social Prescribing colleagues and primary care professionals, and it is essential that you are able to communicate at all levels across both the health and social care sectors.

You will need to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work.
