Job description

We are recruiting for an experienced, organised and compassionate person to join our Social Prescribing team as a Care Coordinator specialising in working with patients with cancer.

You will work alongside the Medway South Primary Care Network (PCN) team, providing patient coordination and care, and supporting the PCN and practice teams to deliver a high standard of support to patients with cancer.

A key aim of care coordination is to help patients engage more fully in their own health and wellbeing. You will work closely with patients, primary care services and hospital team members, supporting cancer care pathways, and helping patients feel more empowered, make shared decisions and feel actively engaged in their treatment.

This will involve being in daily contact with patients experiencing a range of cancers and being responsible for consulting with them and determining their needs. You will develop care plans, coordinate patient-care services and talk to people about their condition; empowering them to be independent whenever possible, and working with the care team to evaluate interventions.

You will also support and signpost patients over the telephone, progressing their care pathway in liaison with other colleagues in the PCN and departments in the hospital.

Quality, individualised care relies on the seamless integration of services around cancer patients, so you will coordinate interventions, and ensure records and information are updated and shared effectively. You will also out audits to review and promote best practice, and liaise with Cancer Research UK regarding populations and services.

Applicants should have experience of working with people experiencing cancer and cancer care, and be able to support people in a way that inspires trust and confidence. You should have excellent communication skills and be able to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries.
