Are you ready for a new challenge?

We are looking for an experienced manager with a background in Health or Social Care to lead our Social Prescribing team working with patients with complex needs, helping them to access the support they need to improve health and wellbeing. Our high quality services strengthen the links between Primary Care Networks, Integrated Community Teams, Community Hospitals, Health Visitors, other providers and the Voluntary Sector.

You will develop staff, oversee day-to-day operations and manage service performance with a strong focus on outcomes. You should have proven experience of managing people and projects, project development, and of multi-disciplinary working. As a manager, you will demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work.

You will be required to provide essential cover where necessary, and assist the Heads of Service in developing the service.

The service is currently delivered across Kent and Medway. The post holder will be based in Medway and must be able to travel to other areas, so a car driver is essential.
