Mental Health Connector x 2 – Mental Health Connector Service   

North & West Birmingham    

£27,085 (£25,279 pro rata) per annum   

Hours: FTE 35 hours per week  

The contract will be until May 2025 with a possible extension for 2 years after this date.

We will be informed of the possible extension by the New Year.

The Service  

  

The main aim of the Mental Health Connector Service is to work as part of a collaborative partnership staff within primary care setting(s). This service is an integral part of local Primary Care Networks (PCN) across Birmingham.  

  

The service will see a team of Mental Health trained professional, Local Authority, Education and voluntary sector staff working together to delivery liaison, consultation, and   intervention delivery. The service is an all-age service and will be based in a primary care setting.  

  

The Role   

  

The Mental Health Connectors will work in a team providing support to Service Users who are experiencing poor mental health and wellbeing. You will work collaboratively with clinical staff and Service Users to support their recovery; action planning and signposting to relevant agencies.  

 

You will input activity and report against key performance indicators and outcomes in line with service level agreements and requirements from the Senior Mental Health Connectors and Service Manager.  

 

You will work jointly with them, attend team meetings, service reviews, individual supervisions, and appraisals.   

 

You will network and liaise with community assets to grow the range and number of support agencies/organisations to ensure that each area is represented, and Service Users have choice.
