Recruiting for a Support Worker- to join our Mental Health service in Hammersmith and Fulham!
GBP 22,568 per annum, working 40 hours per week, plus 25 days Annual Leave, Pension schemes, Staff discounts, healthcare and more.
Support Workers within Hammersmith & Fulham Mental Health Service support customers to help them to develop life skills and increase independence. A wide range of co-produced activities take place across the service, designed to empower and improve the daily living skills of customers. Support Workers in the service work across all 4 projects; working with customers with varying levels of support needs. Many of the customers have complex needs including dual diagnosis and support workers are expected to liaise with other agencies within the borough in order to best support customers.
If a need arises, deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer
Support customers to maintain their tenancies
Support to budget and prevent rent arrears
Assess their practical and emotional needs to incorporate in their support plan
To develop support plans and risk management plans
Carry out weekly health and safety checks
Carry out security duties / welfare checks to ensure the safety of the customers and premises
Monitor the CCTV throughout the shift
Carry out support duties to enable customers to integrate into the community including e.g. accompanying the customer to attend benefit agency appointments and meetings, go shopping, to enquire at colleges about possible courses, to visit various public amenities
- Report any observations relating to customers welfare
Adhere to all the policies and procedures of Look Ahead Care, Support & Housing plus those specific to the project including any statutory requirements
Ensure Look Ahead Health and Safety policies and local protocols are adhered to at all times
Maintain records as required at the project under the direction of Management
Participate in team meetings/reviews and the general development of the service
Undertake any other duties consistent with the grade and nature of the post as may be reasonably assigned
Build relationships and work with other agencies and stakeholders to provide person centred support to each individual

Skills, Knowledge and Experience:
NVQ Level 2 or equivalent or experience within charity/social care sector
Experience with complex needs 1 years- MH and Complex. Adults- would accept young people MH experience.
