Manor Gardens Welfare Trust is a charity with an excellent history of supporting people to improve their health and wellbeing. We operate across London with our main base in Islington.

We work in partnership with the Primary Care Network and Help on Your Doorstep to deliver social prescribing in the north of Islington. The pool of Link Workers, drawn from different organisations, are able to share expertise, knowledge of local services and community assets, and provide choice to residents, informed by people’s needs, feedback and experience.
As one of the Social Prescribing Link Workers in the integrated team, you will be responsible for directly supporting people in linking and navigating to activities, support and services, delivered by us and others, in the Borough. Through your casework, people will be supported to make real change and improvement in their lives, health, and wellbeing, working with colleagues to remain familiar with the local offer for social prescribing and continue to develop and deliver this service into the community.
As the Social Prescribing Link Worker, it is essential that you have excellent interpersonal skills, a compassionate and professional approach to the complicating factors and wider determinants of health that are experienced by many in our communities. Commitment to working collaboratively, a practical working knowledge of IT and electronic databases, and the ability to maintain records of casework, monitoring information and reporting requirements is part of this role. You will have experience of working with communities to engage and support them.
