28 hours per week, 12-month fixed term contract with possible extension, subject to funding
We are seeking a Social Prescribing Manager to nurture and further develop our expanding Social Prescribing service.
Over the last two years, we have worked through partnership to support local people to take an active responsibility for improving their own health and wellbeing by ensuring that the intervention best meets their individual bespoke social needs.
In this position, you will be responsible for the management, development and operational delivery of our Social Prescribing Service which is due to be rolled out borough-wide. You will lead on building and maintaining partnerships while responding to emerging knowledge on social prescribing and working closely with the new Primary Care Networks in Merton.
To succeed, you should be proactive, have experience of leading and developing person-centred services and the ability to develop projects or services from inception to delivery. You’ll also need to be solution-focused and enjoy being at the forefront, actively moulding and shaping an ever-evolving service. A demonstrable understanding of social prescribing and its potential is essential, as is experience and knowledge of the voluntary sector.
