Job description

Mind in the City, Hackney and Waltham Forest, working alongside East London NHS Foundation Trust (ELFT) and other local voluntary sector providers, is recruiting a new and exciting Community Connector role. 

The Community Connector will be working as part of the local neighborhood multi-disciplinary mental health teams, alongside other professionals from primary care, secondary care and the voluntary sector. 

You will be supporting Multi-Disciplinary Teams across the borough to manage the social welfare claims of their clients. This will be as an expert advising other professionals on their queries, as an advisor working one-on-one with a caseload of your own, and also providing some basic supervisory input to Peer Support Workers working within the team.  

The post is currently being offered on a 12–month fixed-term contract.
