What you'll be doing
You will be working with people on a one to one basis, providing advice and support through home visits as well as in group situations. In order to fulfil this role, you will hold a driving licence and have access to a vehicle for work.
You will help people reconnect with where they live and what’s important to them, helping them understand and manage their health and wellbeing, while reducing loneliness and isolation. You will also work with people aged 50+ to remain living independently at home for as long as they want.

You’ll know about what is going on in a community, how to make connections so things happen and have a passion for learning and listening to people with different needs.
You will need to take up an enhanced Disclosure and Barring Check for this role.
When and where you'll be doing it
You will be able to work flexibly, Monday to Friday, based around customer needs.
You’ll be based in North Somerset, with a focus on Weston-super-Mare and Clevedon. Occasionally travel will be required to other locations in North Somerset.
