Lancashire Mind’s Tenancy Management Service works in partnership with several registered housing partners across Lancashire to manage a portfolio of properties on their behalf and provide tenancy support to people living with moderate mental health conditions.

Your new role
We are looking for a new Tenancy Management Worker to join our small and friendly team to manage a caseload of customers who have complex needs including mental health conditions, substance misuse, homelessness, and a possible history of offending.
Your job will be to work closely with these individuals to promote independent living and help them to achieve and sustain their tenancies. This is a challenging and varied job which is often rewarding, particularly when the people we have supported achieve their own goals and move on to build better lives.
What you will need to succeed
You will need experience of working with people with complex needs ideally in a supported or social housing setting and have the resilience to support people with a diverse range of mental health conditions to overcome barriers to independent living.
It is important that the right candidate is an excellent communicator, able to listen and understand people’s needs but also confident communicating and liaising with other local services and partner agencies to ensure each person has the right support in place.
An excellent working knowledge of safeguarding practices for protecting vulnerable people is also key to this role and the successful candidate will need to be able to demonstrate proven experience of this.
Ideally, an understanding of the issues which arise within the social housing sector or experience working in a similar role would be an advantage.
You will need to be a good team player, with the ability to work well with others and be able to work on your own initiative with minimum supervision.
This is an essential car user role as it requires extensive travel to and from tenants’ properties in Lancashire.

The Assessing Social Worker will be responsible for ensuring the success of TACT's projects and change initiatives. Purpose:
To support Carers to review and reassess their Carers Assessments and Peace of Mind for Carers contingency plans in line with local authority procedures. To provide information and support to Carers to reduce the impact of their caring role.
Specific Responsibilities
- Contact all Carers whose Carers Assessment/POM4C plan is due for annual review.
- Ensure that Carers Assessments/POM4C plan reviews are carried out with at least 90% of Carers who are due for an annual review.
- Consider if a Carer may have substantial difficulty in being involved in the assessment review process under the definition provided by the Care Act 2014 and ensure the Carer has suitable advocacy support.
- In undertaking the Carers Assessment/POM4C plan review support the Carer to identify and communicate whether there have been any changes since their assessment that impacts on their caring role.
- Where there have been no changes since their assessment, ensure the Review Form is complete enabling the Carer to continue to receive the same levels of support.
- Where there has been changes since their assessment, undertake a re-assessment to determine the Carer’s eligibility under the Care Act 2014 using the National Eligibility Criteria.
- Where Carers needs for support meet the eligibility criteria, develop and deliver in partnership with the Carer a care and support plan, that is proportionate to the assessed needs.
- Where there has been changes since their assessment, use the Local Authority Resource Allocation System (RAS) to determine a Carers’ payment. Commission Carers’ payments, explaining the payment process to the Carer.
- Ensure that Carers who are ‘not eligible’ in accordance with the Care and Support (Eligibility Criteria) Regulations 2014 are provided with advice, information, signposting and referral on to meet current needs and prevent/delay future needs.
- Ensure that Carers’ Assessments Reviews and Support Plans are complaint with the Carers’ Assessment and Support Planning Quality Assurance Framework.
- Take a whole family approach to undertaking the Carers Assessment Review, considering whether other Carers, including young Carers, are involved in care giving and ensure they are offered information and referred on for support where consent is given.
- Provide up to date information regarding Carers rights and the services and resources available to support Carers.
- Ensure support offered and information provided to Carers is appropriate for their specific needs including but not limited to mental health conditions, learning disabilities and dementia.
- Make outgoing referrals to facilitate Carers’ access to other local services/organisations where appropriate.
- Work in partnership with the other Review Officers to ensure a co-ordinated approach to annual carers assessment reviews.
- Process notifications such as notifications of death and changes to residential status.
- Maintain accurate case recording using both Local Authority Case Management Systems and internal recording systems in line with business processes.
- Ensure any changes made to the POM4C plans are registered with the call monitoring provider for the POM4C Scheme.

We offer individual one-to-one interventions and sessions such as social anxiety, anxiety, finance and budgeting, positive self-image, building self-esteem, support with diet and nutrition, healthy eating, sleep hygiene, DBT skills, building resilience, life skills, solution-based therapy, stress management etc. We offer a range of activities in our service planner including a daily coffee morning, a ‘here and now’ group, community gardening, cooking, arts and crafts, social inclusion and family and friends’ days. We support our residents with daily living skills where this is needed, such as support with shopping and cleaning tasks, this could include prompting or supporting alongside them. We also work closely with other agencies and our residents wider care teams, so good communication skills are required. And of course, all of this is recorded on our information management systems, so good record keeping and documentation is also essential. Our service consists of 8 24/7 Recovery Workers, a Service Manager and Registered Manager.
We know that recovery can look and feel very different to each and every one of us. But we also know that if we provide the right support, at the right time, we can inspire recovery nationwide and that recovery is possible for everyone. That’s where you come in.
Responsible for helping and inspiring the people who use our services to achieve independence in the community, we’ll rely on you to work with them to come up with a person-centred support plan that will see them achieve their goals and aspirations, as part of your key worker role. Whether it’s developing their domestic and finance management skills or accessing work or volunteering or educational opportunities, one thing’s for sure ‐ you will have every chance to shine.
Care home, mental health support or social care experience is preferred and you will be required to administer medication and complete a medication qualification and the care certificate. Candidates with an understanding of mental health issues and/or lived experience would be welcomed. More important is your caring and compassionate nature and empathy and enthusiasm for helping others. Whatever your background, you’ll need to be happy to work both independently and within a team and willing to be part of a weekly rota system and available for on call duties as well as completing sleep in shifts.

We have one permanent full-time role for 37.5 hours per week and one fixed term part-time appointment ending on 13 January 2025 for 30 hours per week. The post holder will be required to work a minimum of one sleep-in shift per week.

From 1st June 2024 Richmond Fellowship will be merging with the mental health charity Humankind to form a single organisation that provides the joined-up mental health, housing and drug and alcohol support we’ve all known has been needed for decades.
