Support Coordinator
We’re looking for an innovative, passionate, and professional individual with excellent communication and organisational skills to join our Stroke Recovery Services based in the Cornwall area.
This is an exciting opportunity to work with stroke survivors and their families to provide Key Worker support following a stroke.
Position: S11121 Stroke Association Support Coordinator (x2 posts)
Location: Home-based, Cornwall however, extensive travel will be required as part of this role (May include team meetings or other work-related meetings)
Hours: Full and part-time (x1 – 35 hours per week post and x1 32 hour per week post)
Salary: Circa £25,500 per annum for full time hours, pro rata for part time hours
Contract: Our services are contracted, we currently have funding for this contract until 31 March 2026.

The Role
The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes including Communication support.
About You
The Stroke Association Support Coordinator will:
· Support new stroke survivors and their carers from hospital discharge into the community.
· Provide personalised information, advice and support.
· Support stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
· Work collaboratively with NHS colleagues and other areas of the community to make a difference in the lives of people affected by stroke.
This role requires extensive travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role.

We are recruiting for a Registered Nurse to join our Bradford IPU on a Bank (as and when) basis.
Hours: Days 0730am-2000 & Nights 1930-0730am
As a Registered Nurse, you'll be part of a multidisciplinary team, working toward providing holistic, person-centred, end of life care, including assessing and understanding the care and support needs of patients and those important to them, planning and providing person centred care based on individual needs.
You will research and network across the system to understand the wide range of services available to patients and those important to them and work with other providers, supporting the wider system to improve the experience of dying, death, and bereavement.
You will assume responsibility for the assessment, planning, implementation, and evaluation of evidence-based, high quality person-centred nursing care for people living with a terminal illness and those important to them.
What we're looking for:
- Registered Nurse on NMC Register
- Experience in NHS or other relevant care setting, ideally in palliative care
- Ability to work in a complex and stressful environment and demonstrate appropriate coping mechanisms
- Be person-centred and compassionate when caring for the person and those important to them
- Ability to develop, implement, and evaluate a care management plan that meets the complex needs of the person, and those important to them
- Proven understanding the barriers and enablers to good communication and how challenges can be overcome
