Job description

Equinox Care provides support, care and recovery services to people who have a range of complex needs such as substance misuse and mental health issues.

We operate across the South East of England, providing a range of different services that concentrate on assisting our service users to re-engage with the communities around them and to find effective strategies for dealing with the challenges facing them in their lives. These services include detoxification services, supported housing, outreach and street engagement.

We are currently seeking a Personal Health and Wellbeing Practioner to work within our Brent Equinox Services to provide a responsive and enabling service for people with complex needs in line with agreed Personal Care plans, by offerring practical based support for people with substance misuse issues and mental health.

Main responsibilities will include:

  • The development of Personal Care Plans, Recovery stars and Risk Assessments 
  • Monitor any changes in behaviour and use appropriate interventions and support
  • Planning and participating in meaningful activities with Service Users to promote their health and wellbeing, positive engagement levels and helping them to live happier, healthier lives. 
  • Deliver group work, psychological and mental health/ substance misuse interventions
  • Provide person centred practical and emotional support, and practical therapeutic interventions for service users. 
  • implement the service’s policies and procedures in order to deliver the overall operational programme. Perform and/or assist services users to perform basic personal care tasks as outlined in the service users personal care plan and recovery star, ensuring that they are supported to self direct the care and support offered
  • Practitioners will encourage service users to make choices about their care and support and ensure that these are communicated to care coordinators, care managers and others involved in the care of the service users.

What we are looking for in the successful candidate:

  • Experience of providing responsive personal care and support to individuals with complex needs
  • Experience of monitoring/supporting service users taking medication
  • Experience of progressive service user provision both in substance misuse and mental health. 
  • Experience of promoting diversity, dignity and equality.
