Job Description

Phyllis Tuckwell Hospice Care provides supportive and end of life care for people living with an advanced or terminal illness. Based in Farnham and Guildford, we serve a population of 550,000 across West Surrey and North East Hampshire.

Night Senior Registered Nurse, Hospice Care at Home

Vacancy = 4 nights per week

Full and part time hours available. Hours: 10pm – 7am

Salary £35,029 for 37.5 hours p/wk pro rata p/a plus unsocial hours enhancements  

Phyllis Tuckwell is looking to develop the Hospice at Home Service at night providing responsive night visits with one of our Health Care Assistants between 10pm – 7am for assessment and care delivery, for patients with advanced illness and deteriorating health under the care of Phyllis Tuckwell Hospice Care at Home team. 

Based on the In-Patient Unit at Phyllis Tuckwell Hospice, you will be responsible for responding to telephone calls from patients and their families known to PTHC, as well as undertaking community night visits to patients known to Hospice Care at Home. We are looking for a proactive experienced nurse who is familiar with working  in the community, has experience of end of life care, and who is passionate about working in an integrated way with the out of hours community nursing teams.

The successful candidate will be a registered nurse and have:

Excellent clinical and assessment skills

Excellent communication and interpersonal skills

Clinical and symptom management experience and knowledge

People and risk management skills

A passion to drive forward professional standards
