We are seeking to recruit an experienced registered nurse with community palliative and end of life care experience and excellent communication skills, to join our Hospice Care at Home team as a Team Lead. The clinical part of this role will involve direct patient care with your team, plus rotation into the co-ordination role where you will be receiving referrals, clinically prioritising and liaising with community nurses, patients, and carers to deliver a high quality and responsive service.

This is an exciting opportunity to further develop your career in end of life care. An informal visit is highly recommended to give you the opportunity to meet the team and ask any questions..
