Prospectus is delighted to be partnering with Age UK Kensington & Chelsea, an independent, local charity supporting older people to remain connected within their community. They offer a range of different services designed with the views and preferences of their service users at the forefront; from Integrated Health Teams and Social Prescribing, to befriending, to dementia support, to combatting loneliness and isolation, to Information and Advice, practical help and homecare, to Digital Inclusion. They also offer Social Prescribing to people over the age of 18 via their trading arm, Age United. They are now recruiting for an Operations Manager to oversee their integrated health and social prescribing services.
Reporting up to the Director of Services, you will provide leadership across two of Age UK K&C's teams; My Care My Way (MCMW), an exciting integrated care service, providing additional support for older adults in GP practices, and Social Prescribing Link Workers (SPLW), a new community-connector service for over 18s embedded across five Primary Care Networks. You will ensure that outcomes for both services are achieved through effective management practices and will motivate and support staff to build long term and sustainable relationships within the GP practices in which they are located. You will work alongside the Director of Services in providing strategic oversight for both services, and will contribute to the development and maintenance of systems to improve service delivery and ensure consistent quality of care.
To apply for this role you will have significant experience of managing and developing frontline teams across the health, social and voluntary sector. You will have significant experience of working with and supporting older people, as well as an understanding of the health and wider social issues affecting them. You will have demonstrable experience of working within primary care, and alongside clinical leads and senior staff. Overall, you will be a strong project manager, skilled at monitoring and evaluating services, and working to ensure the highest quality of service provision for all beneficiaries.
