The Locality Manager role:
- Ensure personalised support is delivered that reflects the needs, wishes and aspirations of the people we support through person-centred care plans and health action plans that are regularly reviewed and implemented.

- Ensure the health and wellbeing of the people we support, in line with CQC Guidelines and Discovery' policies.
- Deliver effective risk assessments and emergency plans to ensure the people we support are safe.
- Make sure your team are appropriately trained and motivated to provide high quality support.
- Ensure the service meets all organisational and statutory requirements and complies with CQC Guidelines and our own standards, policies and procedures.
- Make sure complaints are dealt with in line with our policies.
- Accountable for the budget and finance for the Locality.
- Able to support the organisation to grow the business.
- Develop local connections and partnerships to meet people we support be involved and engaged within their community.
About you
Most importantly, you want to make a difference to the lives of people we support with learning disabilities or autism. To do that, you’ll have:
- A strong background in working with adults with learning disabilities and/or autism.
- Up-to-date knowledge of the personalisation agenda and Active Support model.
- Completed, or keen to undertake, Management Development training.
Beyond those qualities, you’ll also be:
- An excellent communicator, looking to mentor and motivate your team to provide person-centred support.
- Able to prioritise your workload, delegate tasks and meet deadlines.

As a Support and Advocacy Officer you will join a skilled team who collectively provide a unique UK-wide support and advocacy service to over 1500 members.
You will primarily provide direct support and practical guidance to our Fabry members. This patient group is largely adult focused but there will be elements of family and child support. You will raise awareness within both the Fabry community and external agencies; and contribute to the development of resources, to promote self-advocacy within the patient group. A key objective of the role will be to build a sense of community and identity for our Fabry membership. You will also have the opportunity to facilitate and contribute to project work, including youth engagement, advisory boards, and support groups.
In addition to supporting the Fabry community there will also be a requirement to provide support to those affected by other conditions under our umbrella.
Key Responsibilities:

- Develop a good understanding of MPS and Fabry conditions
- Develop knowledge of legislation and best practice relevant to supporting people with life-long conditions
- Provide emotional support, information and guidance in areas such as diagnosis, disease awareness, health and social care and disability benefits
- Support individuals and families to access best possible clinical management and treatment options
- Support and signpost people towards mental health and psychological support as required
- Undertake UK-wide travel to visit individuals and families and attend meetings and events
- Support the provision of online support and advice via social media channels
- Promote awareness by giving talks and presentations to outside organisations, professionals and the general public
Experience & skills
- Minimum of two years’ experience of working with individuals with disabilities within health and social care, education, or related field
- Ability to communicate accurately, concisely and with clarity
- Highly organised with the ability to manage multiple tasks
- Strong attention to detail and commitment to upholding best practices
- Excellent IT skills to maintain accurate case recording, produce clear reports, correspondences and presentations
- Collaborative mindset and enthusiasm for working in a small, dedicated team
- Experience of managing projects and service development is advantageous
Prior knowledge of Fabry Disease is not required as in-depth support and training will be provided. Regular supervision and a supportive work culture will help you to expand your knowledge and become an expert in the work you are undertaking.

We are seeking an Advanced Recovery Practitioner to support in the Women and Families team, within Mind in the City, Hackney and Waltham Forest, working in partnership with Turning Point and Antidote. The successful candidate must have demonstrable experience in working within a substance misuse treatment and recovery setting, must have specialist training and supervision in one or more complexities alongside substance misuse. This may be mental health, long term conditions, criminal justice, offending behaviour, homelessness, motivational interviewing, safeguarding, family work. Have an understanding of risk and risk management, as it applies to substance misuse and complexity. Have a working knowledge of Safeguarding of Vulnerable Adults framework, the ability to follow procedure and development of a culture of safe practice. Cranstoun are proud to work with the West Midlands Police Crime and Commissioner to deliver the Cranstoun Arrest Referral Service within the custody suites of West Midlands Police, and West Midlands Courts, offering drug and alcohol using offenders the opportunity to break the cycle of substance related crime.
Are you currently working as a substance use worker, probation officer, or could you bring transferable skills and experience to our team? Are you a good listener? Can you motivate people to change? You will work between police custody and criminal courts. You will assess, advise and refer offenders, providing them with the opportunity to access treatment to address their substance use. Working with courts, you will promote the use of Community Sentence Treatment Requirements (Drug Rehabilitations Requirements and Alcohol Treatment Requirements) amongst all court professionals and the judiciary, contributing to Pre-Sentence Reports and advocate for our service users.
You will be responsible for delivering all elements of substance use work, including assertive engagement, harm reduction support (including naloxone provision), as well as other associated duties. You need to have an understanding of the issues faced by people who use drugs and alcohol, and who commit crime, and be dedicated to delivering a service that supports these individuals to make positive changes to their behaviour, health and well-being.

About the role
You will be expected to lead and deliver a proportion of day opportunity activities as part of the planned programme and as an opportunity to provide practice leadership to the wider team.
