Job description

Link Age Southwark is a vibrant local charity which works to ensure that older people and people living with dementia can thrive in life. We work with our dedicated volunteers to provide the support people need to live happy, healthy lives. We support around 600 older people and people with dementia each year, and through 400 volunteers, help them stay connected with their communities, alleviating loneliness, improving health and wellbeing and making communities stronger through volunteering.

Our charity is in an exciting period of growth as we respond to greater demand for our services from people with dementia and older people living locally.  We are seeking a capable and positive person who is skilled and experienced in supporting older people and people with dementia, passionate about our charity’s mission and values, and able to work well as part of our small, friendly and professional team. 

Purpose of the Job

Our Service Coordinators play a key role in delivering our high-quality services for older people and people with dementia living in the London Borough of Southwark. 

You will be responsible for assessing the needs of individuals at home and connecting them to our services including volunteer befrienders, exercise and activity groups, and transport for people with mobility problems.  You will coordinate the delivery of at least one of our regular exercise and activity groups in community settings across Southwark, annual social events and outings, and a regular newsletter.  You will keep and use accurate records of our services on our electronic database and help maintain positive relationships with our external partners in the local community.   You will maintain our signposting pack and support your team and service users to access local services.  As the first port of call for many of our service-users, you will provide a friendly, supportive and professional service.
