You’ll be responsible for building excellent rapport with students and working with the Head of Student Wellbeing, Quality and Impact to develop support plans and resources for the students to be their best self.

Career Pathways equips young disabled people with knowledge and confidence. The service enables them to fulfil their career aspirations and personal goals.
As a Careers Adviser you will be giving careers advice to young disabled people in schools across Leeds, at our office and online. We are offering a hybrid model of delivery so working from home for part of your week is available.
About you
- You will have experience of supporting young disabled people to build their confidence and work towards their goals. This could be in either an education or social care setting.
- You will have the confidence to deliver 1:1 and group sessions. These will take place in person and online.
- You will be able to produce clear and relevant action plans. These action plans will aid customers to explore and achieve their career goals

At Supportability we are a charity providing opportunities and support for disabled adults and children to enable them to lead the life they want. We focus on valuing people, teamwork, being competent and responsive and these values underpin everything we do to make life better for the people we support.
‘Celebrating Supportability at 70’ is a Heritage Fund funded, fixed term project that investigates, describes and explains the heritage of Supportability to the people we support and the wider community.
Do you have what we are looking for?
The role will include working with volunteers, people we support and their families and the wider community in order to examine, preserve and interpret significant documents that relate to the charity’s 70-year history. It will also include researching further into other local archives and recording memories and experiences of both past and present stake holders. This will then be pulled together into an exhibition detailing the full history of Supportability, which will take place at a local library. Learning and historical materials from the project will also inform a digital timeline (web design/development will be externally commissioned) hosted on our website.
To ensure the project is accessible to the people we support the role will also involve organising (alongside our service managers) inclusive heritage themed events.
We aim to give people in the local community and beyond a greater knowledge of the history of disability support in Stockport and the part Supportability has played in that journey. Ultimately the project will be evaluated to ensure the outcomes and terms of the Heritage Lottery grant are met.

You will provide clinical leadership to Psychology trainees, ensuring accountability and effective communication with other teams, creating a cohesive and collaborative multidisciplinary therapy team.

About this role:
As a member of YVC’s team of Health and Wellbeing Facilitators you will work closely with colleagues from South Tyneside’s Physical Health Hub, and Primary Care Mental Health Team to support people in South Tyneside, in particular those with learning disabilities and those who require support with their mental health, to meet their wellbeing and physical health needs.
You will help people to access the health services and support they need by ensuring people have information that is accessible to them, by supporting services to make reasonable adjustments, and by supporting the delivery of treatment and assessment programmes under the direction of practitioners.
You will also link with Social Care and Voluntary Sector services to ensure that people can access a range of support in order to improve their health and wellbeing. You will ensure that people’s voice is both heard and acted on to improve this support.

Main Responsibilities:
· Take part in relevant supervision, support and training to ensure YVC deliver a consistent and high-quality service.
· You will promote mental and physical health and wellbeing by:
o Getting to know people who access our service and supporting them
to identify the health issues that matter most to them.
o Linking the person with their GP / Primary Care Health Hub for their annual health check/ mental health program.
o Helping people identify and understand their own health, wellbeing and social needs and empowering people around accessing reasonable adjustments to address their barriers to services.
o Aiding the person to understand their Health Action Plan and what matters to me program.
o Promoting people’s self-confidence and develop communication skills around health needs.
o Supporting people to engage with the appropriate services to treat and improve their mental and physical health issues
· Maintain an active caseload. Manage risk proactively. Keep accurate records using our Charity Log database. Maintain confidentiality for our clients.
· Work closely with colleagues with lived experience of learning disability or autism in the Quality Checker Team to carry out NHS Quality Checks across the GP Practices in South Tyneside.
· Assist in the monitoring and evaluation of the service, including the collection of case studies, data and by contributing to feedback and evaluation sessions with the aim of improving the service.
· Assist in the promotion of the service and when necessary, work outside normal office hours. Be prepared to travel within Tyne & Wear.

Person Specification:

Job Title: Northern Ireland Support and Development Worker
Salary: £27,776 pro rata for part time (FTE 35 hours)

Core role
-
To provide information to individuals, families, carers affected by Spina Bifida and/or Hydrocephalus.
-
To provide advice, support and guidance to enable people living with Spina Bifida and/or Hydrocephalus to access the correct benefits to become financially stable, the right services to improve their health and well-being, and to better co-ordinate their support and independence through a variety of methods.

To refer individual members and/or their families/carers to the most appropriate source of expertise and support in Shine’s Health team and other designated leads within the organisation, or signpost to external agencies as appropriate.

To develop age-specific groups and peer support networks which will provide opportunities for people with Spina Bifida and/or Hydrocephalus to connect and interact, enabling social opportunities and to share experiences, support and learn from each other.

To monitor and fulfil any specific activities to meet the funding requirements for the area

To represent Shine at the adult Spina Bifida Clinic in Belfast as required

To work with and develop opportunities for Shine volunteers.

To contribute to the delivery of regional and national programme of events to Shine’s membership, based on identified need and interests, geographical location and age. These will be face to face and virtual events.

To provide information to professionals working with those individuals and families affected by Spina Bifida and/or Hydrocephalus.

To raise greater awareness of Spina Bifida and/or Hydrocephalus amongst professionals, external organisations and the general public through presentations, training and attendance at professional events.

To actively promote membership of Shine to people with the relevant conditions.

To keep clear and accurate records for monitoring and evaluation using our Salesforce database system.

To work with other service, fundraising, marketing, trust and foundations colleagues and Shine’s membership to identify and seize potential fundraising opportunities for long-term sustainability of the work.

To prepare and keep up to date a directory of relevant contacts across the area and to share relevant information with colleagues.

To undertake administrative duties as required of the role.

At least two years’ recent experience of delivering support and advice services

At least two years’ experience of advising on one or more of the following: health and wellbeing; independence; benefits; education and learning; employment; developing friendships and social skills; reducing isolation; social care; housing

Experience of organising activities and events both online and in person.

Experience of developing and delivering opportunities to enable and empower people with disabilities to achieve their goals towards independence.

Your role
The SEND Education Helpline Adviser provides specialist education advice by telephone, e-mail, and social media to families with disabled children in the UK. You will be supportive and show clear boundaries when offering advice within the scope of the organisation and be confident when dealing with complex and challenging callers and conversations. You will keep up to date with Education law and guidance in UK and signpost callers to Contacts online resources or to relevant local national charities and agencies.
What we’re looking for
You will have at least one year’s experience (or part-time equivalent) of working in an advice role of which SEND advice formed a substantial part. You will have experience and a good understanding of education issues that may impact families. In-depth knowledge of the SEND law in England Part 3 Children and Families Act 2014 and a working knowledge of the SEND Code of Practice 2015. You will be able to confidently navigate websites to identify relevant information. You will have experience of using a CRM database to enter client records. You will be flexible and adaptable in your role and enjoy working in a friendly and supportive team.
