Role:                     Field Care Supervisor

Hours:                  30 hours per week

Location:             Islington, London (with travel across London)

Salary:                  £20, 600 per annum – prorated (£25, 750 FTE)

 

We have an exciting opportunity for an ambition and driven health professional to join us as a Field Care Supervisor to help support our Penrose Community Care and Support (PCCS) services.

PCCS is a CQC registered service specialising in working with the most complex and chaotic clients within social & health care settings. This service will provide floating support to clients within their own home who have physical and/or mental health needs. This includes those who are experiencing psychosis, have a substance misuse or a forensic history, and/or display behaviours that challenge.

The Field Care Supervisor will work closely with the Service Manager to establish new relationships with referring partners and help to develop the growth of the service. Day to day, you will be conducting environmental risk assessments and support plans; including initial home and hospital based assessments. You will assist the Service Manager in ensuring that staff are performing safely, ensuring the highest standard of delivery of service, as well as passing on client concerns and attending multidisciplinary meetings as required.

 Key Responsibilities:

  • Respond within 24 hours to referrals for care and support
  • Carry out initial risk and needs assessments in respect of individual service users, either in the service user’s home or, where required, in other settings such as hospitals or residential homes
  • Support the Service Manager and Central Office Team in the development and growth of the service
  • Carry out the on-site observation and evaluation of care worker practice as part of routine supervision, in response to particular performance concerns, or as otherwise directed
  • Work in partnership with service users, their family, carers, advocates, healthcare professionals and others to ensure the seamless delivery of holistic packages of individual care support
  • Be alert to any changes in service users’ condition and circumstances and act appropriately when these change
  • Monitor the service and Support Worker compliance ensuring CQC regulations and company policies are maintained
  • Seek opportunities to strengthen professional relationships with business critical individuals and identify business development opportunities and networks

Experience Required:

  • Qualification in NVQ Level 2 in Health and Social Care or professional equivalent
  • Previous experience within domiciliary coordinating within a health and/or social care environment
  • Understanding of recovery focused service provision / person centred approach for people with mental issues.
  • Respect for people suffering from a range of medical/mental conditions with different backgrounds and beliefs to your own, commitment to non-discriminatory care practice
  • Understanding of the principles of key working, support planning, and risk assessment
  • Caring and compassionate towards people in need of care and support
  • Ability to plan and organize workloads effectively so that customers receive the services that they expect
  • Ability to use own initiative and work alone or as part of a team especially in an emergency
  • Great communication and interpersonal skills with the ability to build and foster key relationships
