We are working with the SWL ICB and local GPs to deliver an exciting new service within Hampton North. The Community Health and Wellbeing Worker will speak to households about health and wellbeing, alongside offering support to connect people to services and activities in their local community. The role aims to empower people to take control of their lives.
It involves working with people who may be experiencing a number of issues, so you will need outstanding listening and communication skills, empathy and enthusiasm. You will need excellent organisational skills, be adept at seeking out information for relevant community services and have the tenacity to help people achieve sustainable improvements in their wellbeing.
Main duties include:
- Managing a caseload of households, providing monthly home visits and providing a continuous point of contact for all members of the household.
- Working with the client to assess their needs, create personalised action plans and locate support within the community.
- Supporting clients to prioritise and address practical, health, social and emotional issues in their lives which affect their wellbeing.
- Engaging with community organisations and specialist services that provide support to ease the pathway for your clients.
- Building and maintaining relationships with primary care networks, GPs, health care professionals and services across the borough.

Public Voice is a partner in the Haringey Advice Partnership (HAP) which is delivering the Information, Advice and Guidance (IAG) service in Haringey from April 2022 to March 2025 with an option to extend for a further four years. The HAP Project Manager is responsible for managing Public Voice’s overall contribution to the Haringey Advice Partnership and project, ensuring Public Voice meets and reports on its targets and outcomes. We are recruiting a Project Coordinator to oversee the delivery and development of a flagship Community Wellbeing Project, which operates in partnership with Brent Council. The project is a Community Hub hosted in a community space in Stonebridge (not in Sufra’s existing premises). Members of the project can access a Community Shop, a Community Café and Kitchen, and holistic support in the form of Advice casework and workshops. The objective of the project is to deliver food aid alongside wraparound holistic support in way that improves the financial, physical and mental wellbeing of its members, who are struggling with the cost of living.
This collaborative project has been made possible due to the support of multiple partners and has already proven highly successful. It is likely to form the basis of a new model of support for people experiencing vulnerabilities that will be replicated across Brent and possibly elsewhere in London.
The Project Coordinator will oversee the delivery of the project, ensuring both the shop and the café operate effectively and in accordance with agreed standards and procedures. The successful candidate will help shape the project in the months ahead, whilst supporting guests, collecting data, and monitoring and evaluating the project. The ideal candidate will have experience working in a busy and varied environment, managing multiple priorities at the same time. They will have experience in a customer-facing role, dealing with the general public and managing volunteers. It would be beneficial for the post holder to have experience supporting vulnerable people with complex needs from a wide variety of backgrounds. The successful candidate will be the first point of contact for members that sign onto the pilot project, and will be representing the organisation to external partners. Pause works with women who have experienced – or are at risk of – repeated pregnancies that result in children needing to be removed from their care. The programme gives women the chance to pause and take control over their lives, breaking a destructive cycle that causes both them and their children deep trauma. We are now working with more women who have experienced multiple removals than anyone else in the country.
The Pause Practice Lead will manage one of our largest teams – six Pause Practitioners and one Coordinator. The Practice Lead will ensure effective delivery of the Pause programme – they will work alongside their team, bringing a wealth of direct practice experience of complex work to maximise the team’s potential.
The Practice Lead will provide high levels of supervision and leadership to team members from a variety of disciplines, as well ensuring fidelity and integrity to the model. They will typically have significant experience in both operational and project management, with an ability to manage complex data and demonstrate impact. Additionally, they will usually have experience of evidence informed approaches.
Key responsibilities
- Effectively lead and coach the Pause Practice to achieve positive outcomes for women - setting vision, targets and priorities;
- Ensure fidelity and integrity to the model through delivery of the Pause Framework elements, for example, assertive outreach with women and ensuring the most effective form of long-acting reversible contraception is in place;
- Ensure effective pathways are established and maintained with key agencies, e.g. sexual health, housing and mental health;
- Ensure an evidence base is applied to practice through assessment of a woman's needs, case formulation, intervention delivery and review;
- Ensure outcomes are achieved and monitored through the Pause Monitoring, Evaluation and Learning Framework and that real-time data is used to improve practice and reported to relevant funders;
- Report to the Local Pause Board and any other relevant funders;
- Manage expenditure of the Pause Practice and ensure any financial spend on women is aligned with their goals as set out in the care plan;
- Ensure the effective management of all statutory requirements related to the Pause Practice at all times, including safeguarding of vulnerable children and adults, and health and safety of staff;

About LMK
Good relationships shape our health and happiness, yet we are taught so little about them. LMK is a registered education charity on a mission to change that. We run workshops educating young people about healthy and unhealthy behaviours, so that they can avoid abuse and thrive in relationships.
Using film and guided discussions, our workshop leaders support children and young people in honest conversations about relationships and leave them better equipped to spot the early warning signs of abuse and inspire them to enjoy healthy, fulfilling relationships. We explore the 10 signs of healthy and unhealthy relationships and include practical lessons young people want to learn about (like consent) and practice scenarios in a safe, supportive and non-judgmental environment.
LMK is a learning organisation and prides itself in supporting leaders to develop their skills. We offer training and Continuous Professional Development (CPD) opportunities, as well as peer-to-peer learning and reflective practice sessions.
Job Description
LMK is looking for passionate and organised educators (LMK Leaders) to run community workshops, aimed at young people aged between 11 and 24. Through engaging, relatable, workshops you will help young people identify signs of healthy and unhealthy relationship behaviours, provide strategies for them to recognise the early warning signs of abuse and give them tools to keep themselves and their friends safe.
The minimum hours required per workshop is 3 hrs. This includes arriving 30 minutes before delivery, 1.5 to 2 hrs delivery and then remaining back 10 minutes and being accessible to young people who would like further information or a one-to-one conversation, surrounding the subject of relationships.
There is also 30 minutes admin, which is usually completed off-site, such as inputting student surveys and a Leader survey.

Knowledge, Skills and Experience

Extensive experience of teaching or working with young people (9 to 24 yrs old) in an educational, sports or community setting.

Minimum 2 years’ experience co-designing, delivering and evaluating group sessions, consisting of minimum, of 12 and maximum of 30 participants.

Experience or co-designing session plans and co-facilitating with others; able to collaborate equally with others.

A strong commitment to young people and knowledge and understanding of issues affecting their relationships.

Excellent communication, interpersonal skills with the ability to create a safe and non-judgmental space, that allows young people to speak freely about their relationships and experience.

Strong emotional intelligence and the ability to deliver sensitive information whilst treating young people’s concerns with respect.

Capable of maintaining confidentiality and professional boundaries with young people, peers, and professionals.

Able to adapt and be flexible in workshops to get the best engagement from a range of participants.

Skilled at facilitating virtual workshops and working with a selection of online portals.

Entitled to work in the UK.

Hold a DBS certificate or be willing to let us undertake a check on your behalf.

Have a recent safeguarding qualification from a recognised provider or be willing to undertake additional training in this area.

Flexibility to work non-traditional, after-school and occasional weekend / holiday hours.

As part of the Getting Help team, you will provide social and practical support to adults with common mental health problems in a range of settings across Heywood, Middleton & Rochdale (HMR). This includes assessment and support planning, and community outreach. Typically, Community Engagement Workers support people who are on the waiting list for or who are engaging in Talking Therapy. The Getting Help team works in partnership with HMR Talking Therapies delivered by the Big Life Group and Pennine Care NHS Foundation Trust.

Senior Youth Participation Officer
Fitzrovia Youth in Action is looking for an experienced, passionate and enthusiastic person for our Senior Youth Participation Officer role. If you feel like this is you, we would love to hear from you!
The post holder will coordinate and develop our new young people drop-in programme and additional youth action programmes, aimed at engaging local young people into the work of FYA. The successful candidate will oversee the development and delivery of the drop-in sessions, as well as deliver some youth led social action programmes. The role will include recruiting young people, engaging them in co-producing fun and creative activities and workshops which will take place at the drop-ins, as well as supporting them in planning and delivering the social action programmes.
Typically, sessions will occur Sundays (TBC) and young people from the ages 8 to 18 will attend. We are also planning to start a second drop-in session from September, which is likely to run on a Friday evening.
The post holder will support young people to plan some social action programmes directly, as well as support young people into other youth-led community action programmes at FYA.
Examples of projects include youth-led football tournaments, street parties and festivals and peer education activities.
This is an exciting opportunity for FYA as we are elated to have our first drop-in session at our Warren Centre, as we want to reach as many local young people as possible and provide opportunities of social action and support.
