St Martins is a specialist provider of supported accommodation for people who have Mental Health needs complicated by other areas of need, such as from learning disability, drug and alcohol addictions and forensic histories.
Based in St Martins’ Head Office in Islington, the function of the Assessment Coordinator & Practitioner will be to process referrals into St, Martins, to arrange and carry out assessments at given locations, to write detailed reports and to provide follow up customer care to both people referred to St Martins and to statutory professionals that have been involved in the process.
There will also be a role for the Assessment Coordinator within the collation of clinical audit data for the organisation and participate in reviewing the quality of our services.
Key Responsibilities and Tasks Include (but not limited to):
To receive referral information and determine eligibility for St Martins, liaising with Operational Managers and working closely with the Project Managers to identify suitable placements.
Prioritise, process, and respond to all referrals and enquiries submitted by telephone, email or the website.
Attend and participate in meetings within the organisation as appropriate and as agreed by the Operations Manager
