Assessment Practitioner (12mth FTC) 
Penrith, Cumbria 
£32,548 per annum 
37.5 hours per week

We are currently seeking an Assessment Practitioner, with experience within a mental health or social welfare setting, to support the development of a practitioner led assessment service. Based at Jubilee House in Penrith, Cumbria, you will be joining our team at an exciting time for us as we develop our services to provide lifelong support and empower people to achieve mental, physical and social wellbeing throughout their lives. At The Fire Fighters Charity, we exist to support serving and retired fire and rescue service personnel and their families during their time of need. We recognise that no two people are alike, so work in partnership with our beneficiaries, tailoring our services to meet their individual needs to ensure the best possible outcome. 

This is a 12 month fixed term contract during which a responsive practitioner led assessment service is developed and successfully implemented. 

Working in partnership with Service Leads and Lead Practitioners, you will join an existing Assessment Practitioner to develop, deliver and evaluate our assessment service. Acting as a first point of contact for beneficiaries accessing our services, you will carry out telephone and online holistic assessments to identify needs and work in partnership with our beneficiaries and the wider practitioner team to identify appropriate support to assist beneficiaries in their recovery. 

To be successful, you will: 

- Have experience of working in health, mental health, social care or information and advice settings 
- Hold an appropriate registration as a health, mental health or Social Care practitioner 
- Have experience undertaking holistic and recovery focused assessments and planning person-centred support 
- Have experience of working collaboratively with a multi professional team 
- Have an understanding of issues affecting individuals with a broad range of psychosocial, physical and mental health concerns 
- Have excellent IT and communication skills
