At the Together Trust we believe everybody deserves an equal chance in life. There are no exceptions. We provide a wide range of support services including fostering, residential, community and family support. We also provide specialist educational support through our schools and colleges.
At our Specialist Day Provision in the Stockport area, we are committed to supporting adults to live happy, safe and fulfilling lives. We provide person-centred, creative, fun and professional support services to adults, providing them with opportunities to develop their social lives, building new interests and confidence.
We support them to achieve their goals through actively implementing plans that support the development of communication skills, enable independence and build meaningful community integration and leisure opportunities.
As our community support worker, you will…
Working alongside our existing team, you will offer a diverse range of exciting and varied activities for people aged 18+. The work you deliver will be based on a person centred framework and will focus on developing community integration and independence skills in order to help reduce barriers. Facilitating socialisation and developing meaningful relationships is a key element to the role.
As a community support worker you will be actively involved in supporting individuals to live a full and active life. You will do this by ensuring that you and the team around the person do all you can to deliver the agreed plans. You will play an important role, alongside your line manager and senior managers, to ensure that the team functions effectively and is supported through the attendance of productive team meetings and 1-1 supervision sessions.
Community Support Workers play a key role in developing relationships between the person supported, other important individuals in their lives and the Trust. You will also be expected to work closely with any family and professionals involved in the individuals care and ensure that needs and interventions are explored and acted on within your remit.
