Inpatient Discharge Peer Support Worker
Salary: NJC Scale Points 7-11, starting at £20,092 pa
Hours :37 hours per week x2
Job Type: Fixed term for up to 12 months
This is an exciting opportunity to work in a service delivered by Touchstone, in partnership with South and West Yorkshire Partnership NHS Foundation Trust and Kirklees Council. The project is funded by the NHS Kirklees CCG to improve sustainable outcomes for people who have been discharged from inpatient Mental Health Wards. The development of a sustainable pathway focuses on a multi-agency approach involving clinicians, health workers, housing support workers and peer support workers; to work in a person-centred way. The aim is for people to develop skills to become tenancy ready; build upon strengths and to work together to avoid repeat admissions to hospital, with the aim of avoiding delayed transfers of care from hospital. Touchstone will provide the peer support element to the pathway and staff will work with partners to deliver the pilot.
As a Peer Support Workers (PSW) you will work within the Discharge Support Service, supporting people who are leaving hospital inpatient care, to overcome barriers to discharge and reduce re-admission. This will involve securing housing and accommodation and offer other advice around benefits, debt, and other identified needs. Each peer support worker will be based on one of three inpatient psychiatric wards, and they will alongside integrated Community Mental Health Teams and housing workers to provide a holistic approach.
