Care Coordinator - TEMP
Our client a London based Hospice are seeking an efficient and professional Care Coordinator to join the team on an ASAP basis for 2-3 months
The End of Life Care Coordination Service aims to improve the coordination of a wide range of care provided to end of life care patients and their families in Wandsworth. The team is made up of both clinical and non-clinical roles. The goal of the end of life care system is to support patients to achieve their preferred place of care and death, to avoid unplanned and inappropriate hospital admissions and to support rapid discharges from hospital. This role plays a vital part in achieving this.
You will be working as part of a team under the direction of the Lead Coordinator. However, you will need to use your own initiative to liaise with end of life care patients, their families and healthcare professionals with the key aim of improving access to and coordination of services provided by other health, social and voluntary care agencies.
The nature of the role may involve interactions with distressed and anxious patients and their cares and families.
Knowledge, Training and Experience
- Strong attention to detail
- Ability to work alone
- Able to manage and communicate rotas internally and externally
- Advanced IT skills
- Skilled note taker
- Working knowledge of all computer databases and willingness to learn Crosscare and Coordinate My Care
- Working knowledge of all relevant organisational policies
- Understand the full range of processes and procedures within the Care Coordination Service to ensure efficient and effective working
Role: Care Coordinator
Employment Type: Temporary 3 months +
Salary/rate: £11.30ph - £11.94ph (£22,028 - £23,277pa)
Shift working full-time, 150 hours per four-week period worked in thirteen 12-hour shifts from 8am to 8pm and one 7 hour shift. Inclusive of weekends and public holidays.
