The Role
Our Practitioner Led Assessment Service is the entry point for our beneficiaries seeking support from the Charity. Our Assessment Practitioners work in partnership with our beneficiaries to identify goals, individual needs and appropriate solutions.

The team provide advice, information and support and work collaboratively with the multi-professional team to ensure our beneficiaries are offered the right support at the right time.

Reporting into the Beneficiary Experience and Continuity Lead you will be responsible for day-to-day management of the practitioner led assessment service, holding a small case load and reviewing and resolving complex cases with the team. 

You will work collaboratively with key stakeholders across health improvement and rehabilitation services, to ensure services are integrated, person centred, outcome focused and based on an ethos of recovery.

About You

Job Title: Caring Dads Practitioner

Hours: 22.5 hours per week across 3 days.

Caring Dads exists to change current practice to better include fathers in efforts to enhance the safety and well-being of their children. The practitioner will work directly with fathers with an aim to improve relationships with their children, families, and co-parents/carers. This will include promoting the service to a range of teams across Hampshire and Southampton, to generate programme referrals. You will work closely with Hampton Trust domestic abuse perpetrator service to identify client suitability for the Caring Dads programme.

As the Caring Dads Practitioner, you will be required to deliver the Caring Dads and Accredited Domestic Abuse Prevention Training (ADAPT) Programme. This includes one to one support and group work. Groups are delivered on weekday evenings; you will be required to deliver one group work sessions per week.

You will undertake treatment and risk assessments of individuals causing harm in their relationships; this includes active case/risk management of families living with domestic abuse. Although this position is based in Ashurst, you will be co-located into social work teams across Hampshire three days per week, with an aim to providing support and advice in identifying suitable families for the Caring Dads and ADAPT programme.

About the role:

The Foundation/Domestic Abuse Prevention Practitioner (FDAP) will play a key role in working directly with individuals using harmful/abusive behaviour within their intimate relationships. The role will include suitability and risk assessments, delivery of interventions, facilitation of group programmes, risk identification, information sharing and preparation of reports for courts and other agencies.

The FDAP Practitioner will be co-located into Priority Crime Teams situated in Newport Police Station to assist with the identification, screening, and engagement of individuals using harmful behaviour in their relationships, working closely with police to provide a timely and streamlined process for earlier identification, referral, and engagement/disruption of high harm perpetrators of domestic abuse.

For this role is it essential you have excellent understanding of domestic abuse, including the impact on victims and their children. Experience of working with individuals experiencing domestic abuse/ and or causing harm. Experience of group work delivery is desirable; however, we will consider applications from candidates who demonstrate an aptitude for the role.

You will need to be able to build and maintain effective working relationships with statutory and voluntary agencies to deliver the best possible outcomes within a multi-agency setting. As well as being tenacious in support of your clients and adept at problem solving.

IT literacy is a must and candidates must evidence both the ability to use bespoke packages and excellence in their recording practice.

You will work with young people between the age of 17-25 with needs related to substance misuse, including those that are affected by their parent’s substance misuse. The purpose of the role is to engage young people in a variety of interventions, using a young person centred approach, empowering them to reduce risk, increase resilience and achieve positive outcomes.

Based in the Transitions team, you will support young people with multiple risk factors and substance use. Our Complex Needs Practitioners work flexibly across the community and within our multi-agency partnership settings. A core part of the role will be working with young people and their families to address holistic support needs across all aspects of the young person’s wellbeing. We are looking for a full time Peer Engagement Practitioner.

This is an innovative new service, integrating the expertise, skills and knowledge of the network in Barnet with NHS Mental Health services. The purpose of the role is to provide peer led psycho-social support to clients, working within specialist Mental Health teams and the VCS delivery partners. This role will also serve to enhance the work of the MH teams, and other practitioners within the service, to ensure it remains clients focused, recovery informed, and embedded with experience led practice.

Your challenge? To provide advice, assessments, brief and structured interventions and group programmes. As a Practitioner in this role you will be the initial contact for referrals into the service, conducting initial assessments via telephone, face to face and offering relevant and appropriate advice, harm minimisation, family interventions risk assessment and management, actioning onward referrals to partner agencies. You will also be required to manage a caseload.

An energetic and confident self-starter, you have a qualification in health/social care, youth and community work (e.g. NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies). Alternatively, we’ll consider candidates with experience of working in the substance misuse field with a commitment to complete NVQ Level 3 Health and Social Care. As well as good knowledge of alcohol/drug and health related issues, you're used to liaising with voluntary and statutory agencies and health professionals and comfortable engaging with clients in a variety of service delivery settings. The Junction is an out of hours “calming café” service which is open 365 days of the year to support people experiencing pre and/or mental health crisis. The aim of the project is to alleviate the need for people within the Swindon locality to access A&E and/or acute mental health services.

As a Wellbeing Practitioner, you will be providing a preventative, person centred approach to people accessing the service who may be experiencing a mental health crisis.

The service is open from 4PM-11PM and there are positions available for part time hours and full time hours and  working patterns will include some weekends.
