Adult Re-settlement Case Worker – Bridge to Home Project
Hours: Full time 35 hours a week
Contract: 12 months
Salary scale: £25,767 per annum, pay award pending
Responsible to: Housing & Wellbeing Lead
Location: Option to work from the office in Chorley or a mix of home and office working, with extensive travel across Lancashire for work activities, meetings, and training.
Bridge to Home is a support initiative aimed at helping people transition from hospital wards back into their local communities. Our goal is to provide a seamless and comfortable experience for those who have been discharged from the hospital and require additional support during their recovery journey.
Your new role
This role will be responsible for delivering personalised intensive one-to-one support and guidance to individuals accessing the Bridge to Home and Lancashire Mind Housing & Wellbeing services in and around Lancashire
You will be required to work closely with hospital ward staff and other statutory services to offer personalised opportunities to people experiencing mental health issues, ready to be discharged from hospital wards.
What you will need to succeed
You will need experience of working with people a diverse range of mental health conditions to overcome barriers to independent living, to have a knowledge of homelessness and housing needs, specific to people with mental health issues and have experience of collaborating with medical professionals, social services, housing providers and local community and voluntary groups.
It is important that the right candidate is an excellent communicator, able to listen and understand people’s needs but also confident communicating and liaising with other local services and partner agencies to ensure each person has the right support in place.
An excellent working knowledge of safeguarding practices for protecting vulnerable people is also key to this role and the successful candidate will need to be able to demonstrate proven experience of this.
Ideally, an understanding of the issues which arise within the social housing sector or experience working in a similar role would be an advantage.
You will need to be a good team player, with the ability to work well with others and be able to work on your own initiative with minimum supervision.

About this role
The MENA Death Penalty Caseworker will undertake a range of activities in support of Reprieve’s work on cases of individuals on death row or at risk of the death penalty across the MENA region, including casework and investigations into individual cases and the broader death penalty landscape, support to Reprieve clients, and support to partner organisations and lawyers in the MENA region and beyond.
The MENA Death Penalty Caseworker is an integral member of the MENA Team, and will work closely with the Media, Campaigns and Digital, and UK and US Policy teams, and the Directors and Deputy Directors.
Location and salary
This role is a full-time, permanent contract based in our London office. The annual salary is £40,964 full-time per annum, less any required deductions for income tax and national insurance.
Reprieve operates a hybrid working model and we require staff to work a minimum of 40% per week from the London office, and the rest from home. Your presence is important during core office hours, whether remotely or in the office.

The Refugee & Migrant Services Caseworker will join the Refugee & Migrant Services Support Team, who work alongside our ESOL team and volunteers to meet clients’ needs as directed by the R&MS Team Leader. The post is based at our offices in Bedford but will involve some local travel to deliver support and home appointments. Our team deliver a number of services, including:
- Welcoming and providing holistic integration support to resettled refugee families, empowering them to live independently with less support as time goes on.
- Providing vulnerable migrants with high quality information and advice, empowering them to make well-informed choices.
- Providing supported accommodation for people who have been refused asylum.
MAIN DUTIES AND RESPONSIBILITIES
- Building trust and maintaining positive working relationships with all clients, external agencies and other areas of KAP.
- Systematic recording of all notes and actions related to client support, maintaining up-to-date risk assessments and support plans using our case management system.
- Fulfilling funders’ & commissioners’ reporting requirements; generating statistics, case studies and keeping detailed records.
- Meeting resettled families at the airport and bringing them to their new homes.
- Ensuring clients understand their rights and responsibilities regarding their accommodation, supporting them to fulfil these.
- Supporting clients through advocacy to access; health & wellbeing services, welfare benefits, housing, immigration advice, community events, education, training, volunteering and paid work.
- Assisting resettled families’ children to access education and ensuring appropriate support is in place for them.
- Provide advice and support to resettled families through home appointments in Bedfordshire.
- Provide advice and support to clients both remotely and at in-person meetings
ADDITIONAL DUTIES & RESPONSIBILITIES
- Maintaining working practices that reflect the overall ethos and vision of KAP, including following our policies, procedures and code of conduct.

About this exciting opportunity
Embedded within the local authority, our successful candidate will carry out one-to-one mentoring work with a caseload of clients (aged 11-25 years) and produce support and risk management plans based on assessments, ensuring that they are equipped to think critically, assess risk, and make better life choices.

- Experience working as part of a multi-agency team
- A Level 3 qualification in relevant field or willingness to work towards one
- Knowledge of the issues facing this client group – gangs, knife crime, social exclusion and exploitation
- First-class interpersonal, relationship-building and communication skills, both verbal and written.

About the role:
The Non-Crimes Assessment Worker will work within a dynamic, fast paced, crisis intervention, advocacy and support service to ensure the voice of survivors informs every stage of the process. They will work within the team to make proactive contact attempts with a high volume of new referrals. Their main focus will be contacting the police cases that may otherwise not get referred into the service. Once this contact is established complete initial needs and risk assessments. They shall provide initial advice and crisis safety planning before the case is allocated to an IDVA/caseworker or referred to the appropriate partner agency for ongoing support. They will provide this high quality support based upon a client led needs and risk assessment to women from the age of 18 and over who access the domestic abuse service. They will advise women on criminal justice, civil remedies, housing and related matters.
About You:
You will have an excellent understanding of domestic abuse and its effects on survivors and children. Be skilled in risk management and safety planning and is experienced in domestic violence advocacy, who has worked with a high volume of clients who have complex and multiple needs. You will have a countenance that remains calm in a crisis and in handling sensitive information on a daily basis and you will have experience working within safeguarding procedures.

Job Description
The role of Service Coordinator is an exciting opportunity to oversee a team of Caseworkers, ensuring that they provide first class to support, to young people who are at risk of child exploitation, or have for a time been missing from home.
This role will assist the Service Manager to establish strategic links in the area, with key partners and other stakeholders, with a view to promoting and continually enhancing the service. We are co-located within a multi-agency team, tackling missing and child exploitation, so you'll have a great opportunity to develop your networks and expertise within the sector.
The cases that we work on are complex and challenging, but are making a difference in the lives of those that access our services. We've completed over 1500 return home interviews, helping to prevent further missing episodes. We've also supported over 130 young people through a programme of direct work, tailored to their individual needs.

Qualifications
We are looking for someone who can demonstrate the following skills, qualifications and experience:
- Relevant professional qualification in youth/community work, social care, health or education.
- Able to manage across a wide geographical area, ensuring that preventative and reactive services are provided to children and young people at risk.
