Who we are
ABOUT US
- Our vision is 'Healthy, connected people and communities'. We improve health, empowering people and communities to live well and tackling the root causes of ill health including isolation, loneliness, disadvantage and frailty.
- We are a charity established in 1975 to develop volunteering and communities, we are now leaders in social prescribing and other innovative approaches to improve wellbeing in communities.
- We are ambitious about the impact we can have on health, and seek parity of esteem between social and clinical interventions. We believe age is not a barrier to good health, and we work to the World Health Organisation definition of health as "a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity."
- One of our partner commissioners says: "Involve Kent have an excellent knowledge of our client group and are professional and empathetic when it comes to meeting their needs. They understand the importance of a holistic approach and are able to sign post on to other services that may be beneficial to patients when appropriate this includes effectively understanding safeguarding and the importance of this within their work parameters."
SOCIAL PRESCRIBING
- Involve is leading the development of social prescribing in Kent with innovative evidence based programmes, integrated with health partners. We work to the NHS England guidance on social prescribing:
- Social prescribing enables all local agencies to refer people to a ‘social prescribing link worker’ to connect them into community-based support, building on what matters to the person as identified through shared decision making, personalised care and support planning, and making the most of community and informal support.
