Support Coordinator

This is an exciting opportunity to work with stroke survivors and their families to support them following stroke. We’re looking for an innovative, passionate professional, whose values match our own, can think independently and is open to a coaching style of management, to join our Stroke Recovery Service based in Suffolk.

Position: S11157 Stroke Association Support Coordinator – Communication Support

Location: Home-based, Suffolk however, frequent travel will be required as part of this role (may include team meetings or other work-related meetings)

Hours: Full-time 35 hour per week (flexible working available)

Salary: Circa £25,500 per annum

Contract: Our services are project-funded, we currently have funding for this service until 31 March 2025.

The Role

The service aims to identify and address the needs of stroke survivors and carers whose lives are affected by aphasia, by providing a range of innovative support solutions, supporting them to meet their desired outcomes. A key part of the role will be to support service users and the delivery of the service.

Reporting to the Stroke Association Service Delivery Coach, the Stroke Association Support Coordinator will:

· Support stroke survivors and their carers to develop appropriate communication strategies and practices, Build confidence in the use of these communication strategies in order to live life with maximum independence

· Run an effective service for stroke survivors and carers, including face to face, digital and online resources, ensuring that confidential and accurate records are kept on our CRM data base

· Provide personalised information, advice and supportEnable stroke survivors to make informed lifestyle changes which will help them to prevent further strokes

· Work with other health and social care professionals across the stroke pathway to ensure high quality support

About You

Knowledge, skills and experience you will ideally have:

· Background in a caring profession, ideally supporting people with disabilities

· Excellent IT skills and an ability to maintain accurate records

· An affinity with the values of the Stroke Association

· A flexible approach and an ability to effectively manage a caseload

This role requires frequent travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role.

Role Requirements

 As part of the site services team the role holder will be expected to liaise with managers and staff across the Trust. The role holder is expected to work closely with the other members of the facilities team and assist in covering in the laundry, in the deep clean team or other works as and when required.

You will be a natural and enthusiastic housekeeper working not only as part of a team but also individually. You will be health and safety aware, take ownership of your own work, and be understanding of the children’s needs here at The Children’s Trust. Good organisational skills, flexibility and being friendly and caring in nature are a must for this role. Please note that the role requires a significant degree of manual handling and working with houses services and laundry equipment.

With experience of working in a complex environment, across a large and diverse workforce, you will be exceptionally organised with a high-level of attention to detail. Overall Duties of the role will include:

• Being an accessible point of contact for the children living with our carers and to our young people, keeping them up to date with the facilities and opportunities available with TACT
• Providing support to children with emotional welfare and mental health needs
• Supporting interventions and activities to ensure stable placement arrangements
• Being able to transport children and young people to events, appointments and meetings (mileage reimbursed)
• Organising and participating in virtual and face-to-face events and occasional residential meet-ups
• Managing all paperwork associated with events and activities
• Willingness to work flexibly, according to deadlines and needs of our families which may include some after-school, evening and weekend work
• Undertaking trauma-informed direct work with children and young people

Our Advocacy services we provide advocacy direct to children and young people in a variety of situations. Advocates around the country support children and young people to get their voice heard in decisions about their lives. This may be through the telephone helpline or through an advocate working directly with a child, for instance, to support them at a review meeting or to help them make a complaint about their care. Coram Voice provides visiting advocacy services to most of the secure units nationally, to Secure Training Centres, Juvenile Young Offender Institutions, psychiatric hospitals, residential special schools and children’s homes.

About the Role

We are looking for an Advocate to join our team in Swindon. We are seeking applicants who have transferable skills, a willingness to learn, a desire to promote the rights of children and young people or already have an IAP City and Guilds Level 3 or IAQ City and Guilds Level 4 in Advocacy.

You will work directly with care experienced children and young people and those on Child Protection Plans providing them with advocacy support in the community and a variety of settings.

You will empower and support them to ensure their voices are heard within decision–making processes that effect their lives.

You will be a capable ambassador for Coram Voice with the ability to engage effectively with professionals, carers, other stakeholders and most importantly children and young people.

Successful candidates will be invited for interview.  The interview process comprises of a written exercise and a panel interview.  Successful candidates will have a further one to one interview in accordance within Warner recommendations. At TreeHouse School, we're looking for a SEND Officer to join our team.

You'll lead the scheduling and coordination of the statutory annual review process and be responsible for planning, organising, minuting and drafting all annual review paperwork. You'll support the effective provision of the school office administration by having regulatory knowledge to meet the needs of TreeHouse School and work closely with the reception team where required to support a variety of stakeholders.

You'll liaise closely with the school based Transdisciplinary Team staff, including class teachers, OT, SaLT, PBS specialists as well as outside agencies. You'll manage and organise pupil appointments, including informing parents/carers. You'll manage diaries/Outlook calendars, make appointments, book meetings and training rooms, whilst also being responsible for meeting minutes and note taking.

We are looking for someone who has:

• Formal administrative/reception qualification or equivalent experience
• Excellent working knowledge of SEND code of practice and Annual review process
• Experience of working within a similar SEND organisation/school/charity environment
• Experience of using own initiative to plan, organise and manage own workload

This is an exciting role in our Supported Accommodation team that will be the first point of contact for the hostel and input information on the client database in order to ensure that all staff have timely and accurate information about residents. You will also assist the Registered Manager in recording and reviewing documentation in relation to the Supported Accommodation (England) Regulations 2023 and our registration and regulation under OFSTED. In addition to this, you will process voids (vacant rooms) in line with housing management procedures, ensuring that rooms are cleared and liaising with the Facilities Manager to ensure that rooms are habitable as soon as possible.
