Job description

Successfully shortlisted candidates will be interviewed on a rolling basis. 

NHS England describes Social Prescribing as "focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support." 

A key part of the SAIL navigation role is making sure that people are safe and independent at home. For example, Navigators may identify possible trip hazards such loose carpet fittings, or smoke alarms not working etc during a home visit. Navigators will then refer clients to our free in-house handyperson services. Older people may be socially isolated so a Navigator may inform them what social groups are available and help them access it. The SAIL Care Navigator acts as the link between the older person and other services, involving family members, volunteers and partners where relevant to support the implementation of the plan. 

SAIL Care Navigators currently take referrals from the SAIL checklist, visit people at home and use person centred thinking tools to support older people to create and implement support plans to address isolation and malnutrition and any other goals the person wants to achieve.
