The Peer Support In-Reach Service is a partnership between several local Minds. Peer Support Workers and Senior Peer Support Workers, who have direct lived experience of mental health issues, are working on in-patient wards in Croydon, Greenwich, Lambeth, Lewisham and Wandsworth to provide recovery-focused peer support to people as they leave hospital and journey towards being settled in the community and living independently.

South East London Mind have a vacancy for a part-time Senior Peer Support Worker to join the team in Lewisham and line manage a Peer Support Worker.

The successful candidate will have personal lived experience of mental health problems, as well as experience of managing staff or volunteers. You will need to be able to travel around the local area to meet your clients in the community and work in hospitals.

You will use your lived experience to help people build skills to develop independence, connect with family and friends, pursue social or vocational interests, get more involved in their local community and stay healthy and mentally well. Your support will be person-centred and may include mentoring, coaching, facilitating access to community activities and providing practical and emotional support. You will meet people before they leave the ward and work with them to develop their own support plan, empowering them as they transition back into the community.

Responsibilities and Person Specification: 

This role is an integral part of the day-to-day delivery of our Advice and Support Service, which has been re-shaped and is due to go live in the summer of 2024. The service is targeted at older people in or facing financial hardship, and provides information, advice and follow up support relating to a range of issues, including Welfare Benefits, Social Care and Housing.  

As a Generalist Adviser you will be responsible for supporting older people, their families and carers through our duty service with their welfare benefits and housing and other issues relating to financial hardship in later life.  

You should have a good level of knowledge of welfare benefits, social care and housing as well as experience of working within a high-volume advice service, ideally a telephone-based service.

Salary information:

The Programme Manager will work with colleagues to design and deliver our ongoing Experiences of Care (EoC) programme.  The programme has been developed to equip and support staff, patients and carers to improve people's experiences of health and care services.

The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. • To act as a first point of contact for Tower Hamlets residents and professionals via telephone and email

. • To gather information required to efficiently triage customers’ strengths and needs and make appropriate referrals.

• To provide information and advice to residents on local services to support prevention and promote independence.

• To actively promote use of the digital portal and signpost to digital support services where appropriate.

• To maintain excellent records of all contacts and enquiries on all relevant case recording and data systems ensuring accuracy, quality and timeliness while complying with statutory requirements and maintaining confidentiality.

General

• To meet regularly with line manager for support, supervision, and appraisal.

We are recruiting for a Head of Audit and Improvement job share partner to work up to 26 hours per week alongside our existing Head of Audit and Improvement. We can be flexible on days worked and this can be discussed at interview. This role will be mainly working from home with some occasional travel when required. 

In this role you will:

• Lead quality performance, supporting, advising and proactively identifying risk and areas for improvement in each Operational area including adult social care and educational programmes
• Provide oversight for quality frameworks and methodology ensuring currency according to social care or legislative changes.
• Lead a team to ensure the programme of improvement and quality assurance activities are delivered and performance expectations are met.
• Ensure accurate and meaningful information on aspects of quality performance is collated and analysed, to provide assurance and intelligence to help inform decision-making.
• Be an active and supportive member of the Senior leadership team, advising on issues relating to quality
• Design, implement and embed an audit and improvement programme across operational services
• Deputise on occasion for the Executive Director of Quality and Impact

• has experience of working in a similar role, ideally within Health and Social Care
• is a great communicator and influencer and enjoys working with all stakeholders up to and including exec level
• is knowledgeable of our regulatory environment
• has extensive knowledge of quality frameworks, methodology and tools
• has line management experience with the ability to coach and mentor colleagues

ABOUT THE ROLE

We have a new opportunity for a specialist Dual Diagnosis Worker to join the team in Croydon on a 12 month fixed term contract. This is a new role to the team and you will deliver evidence-based interventions based on  holistic assessments to support personalised recovery, integration, and promotion of independent living skills. You will work to the long term support needs of our residents and work towards facilitating improved outcomes. 

Our service has 27 residents across 4 sites, which specialise in Forensic Mental Health aiming to enable and empower our residents to achieve their maximum levels of functional independence and enhance their sense of wellbeing.

Shift Pattern:  This role is full time 37.5 hours per week. Hours to be discussed, you will be based in service.

OVERVIEW OF KEY RESPONSIBILITIES

• Set up realistic and flexible strengths-based support plans, working closely with colleagues and support teams to support and advise on interventions and approaches to meet resident/participant needs.
  Include appropriate substance misuse interventions within support plans for those with co-occurring substance dependence and mental health need devising systematic plans for care; implementing this plan; coordinating the input of others and evaluating effectiveness.
• Undertake joint holistic assessments and risk assessments with residents/participants to help determine service and support needs and identify where these can be met. Provide outcome-based support measured against goals, recovery, and achievements.
• Encourage residents/participants to identify and define their own goals and work towards being resilient and independent as possible, support with reducing reliance on clinical interventions and hospitalisations. 
• Promote active involvement of those with dual diagnosis, assist residents/participants to identify their own strengths, personal interests and goals and support them to access and participate in activities of their choice.
• Undertake time-limited, SMART, and a range of evidence-based interventions such as 1-1’s, including harm minimisation, increasing awareness and motivation through stages of change and relapse prevention.
• Support in creating and maintaining a welcoming, psychologically informed home environment for residents/participants, ensuring a continuous focus on safety, health, and wellbeing.
• Positively promote and support connections with local communities and boroughs of SIG’s services and liaise with other teams to maintain and share knowledge and links to support people to access resources which include print, personal and digital.
• Develop community links and liaise regularly with a range of agencies and individuals across voluntary and third sector services in the community including but not limited to; drug and alcohol services, supported employment schemes, gyms and healthy living programmes, recovery colleges, and benefits and advice agencies.
• Act as a role model for staff of all disciplines and grades working with people with co-occurring mental health, alcohol and/or drug problems. 
• Share knowledge and skills to help enhance awareness around dual diagnosis and best practice, challenges, and services available with other services of Social Interest Group.

Other responsibilities than those described above may be required to be undertaken from time to time and will be expected to be performed to as long as it is within the capability and level of the position. 

KEY CRITERIA

What we are looking for:

• Knowledge, understanding and experience of working with people of complex backgrounds. Ideally ex-offenders, those with enduring mental health, substance misuse and challenging behaviours
• Experience of assessing needs and strengths, and supporting people with dual diagnosis and complex needs
• Previous experience in a similar role, providing holistic assessments and support for people experiencing mental illness who use substances
• IT Proficiency, including Microsoft Office, and the ability to navigate and learn new case management systems and other types of organisational software
• Able to recognise ill health and additional support and need for interventions
• Knowledge of the interactions between mental health problems and drug and alcohol use and the issues facing people trying to stabilise their situation
• Applied knowledge of the principles of risk assessment and risk management, particularly in relation to harm minimisation
• Ability to engage those who may be hard to reach, able to influence and negotiate positive outcomes with others

What we would like, but not essential:

• Experience in delivering training in motivational interviewing, Cognitive Behavioural Theory informed approaches and other relevant recovery models
