Who we are

Age Connects is dedicated to improving the lives of older adults by tackling social isolation, loneliness, and poverty. We provide practical support, create safe spaces, and collaborate with health & social care, GP clusters and third sectors, by integrating our services we ensure the individual receives the holistic support to meet their needs addressing medical and social needs.

Our Core Services

• Broker Service: A trusted single point of access integrated within health & social care teams, providing direct support pulling together a range of services to meet identified needs, a gateway to access all services.
• Good neighbours and Community Support Programs: Volunteers providing befriending , shopping, practical and emotional support when needed.
• Welfare Benefits: Supporting people 60+ to claim state benefits to which they are entitled.
• Nail Cutting: An affordable footcare service that supports health and mobility
• Vale Wellbeing Café: A safe & welcoming space for people to socialise , join activities and improve mental health
• Local Energy Partnership ( Leap) Energy saving guidance helping individuals lower costs and improve home comforts.
