Care Coordinator

Contract: Fixed Term Contract (Maternity Cover) up to 12 months, Part Time.
Hours: 17.5 hours (days can be agreed,  although the successful candidate must be able to work Tuesday am)
Location: Manchester, ( currently working remotely)
Salary: £30,746 pro rata per annum (actual salary £15,373 pa) Grade 18R

We have an exciting opportunity for a Care Coordinator to join our team to support the clinical service delivery. Many of our clients have complex needs and this role ensures that clients are supported to access and engage in multi-agency services.

The role involves assessment, care planning and provision of stabilisation support to our most complex clients. The role involves working alongside Therapists,  Legal and Welfare staff as part of a truly multi-disciplinary team. The ideal candidate will have prior experience of working in mental health / social care settings, and be committed to a human rights approach in working with asylum seekers, refugees and survivors of torture.

In return, we offer a competitive package, with a generous 30 day annual leave entitlement, and 6% pension contribution.
