Oaklands & St Kilda's is made up of two newly refurbished six-bedroom homes, located close to each other in Ealing, West London. We work with up to 12 young people at a time, providing a safe, supportive, and welcoming environment. Many of the young people joining us are moving on from hospital, stepping down from more intensive placements, or transitioning from Child and Adolescent Mental Health Services to Adult services.
Our team delivers 24-hour trauma-informed and psychologically informed support. You'll play a key role in helping each young person shape their own support and service experience. Everyone receives a bespoke welcome pack and is encouraged to personalise their living space, helping them feel at home from day one.
We place a strong emphasis on participation and co-production. Young people are encouraged to make decisions about their care, engage in partnership meetings, and take part in activities that build skills and confidence - whether that's cooking, budgeting, managing medication, or accessing education and work opportunities.

Support workers will manage a number of young people to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.

This is a Waking Night role working to support young people throughout the night at the service.

This role consists of four weekly shifts from 22:00 to 08:00, with double cover for each shift.

In addition to the 50+ young people we see every day out our centre, New Horizon’s Outreach service works with young people on the streets and in the community. The team works pan London to deliver a youth-specific outreach service to young people currently or at risk of rough sleeping. We use our extensive housing expertise to place those young people into accommodation that is appropriate and safe, whilst encouraging them to access the day centre to benefit from the wide range of services we offer. As an Intervention Worker at Hestia, you will play a crucial role in our Highbury Approved Premises. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence. A day will never look the same and some of the things you can expect to do/be responsible for are:
The role in Approved Premises (AP) involves supporting high-risk individuals transitioning from prison back into the community. Staff work closely with residents, providing guidance, promoting positive behaviour, and helping them build independence while maintaining a structured environment. Responsibilities include preparing meals, supporting rehabilitation plans in collaboration with probation and external agencies, and ensuring residents follow rules such as curfews and drug/alcohol testi
ng. Staff are also expected to assist in running and documenting activities, completing daily tasks, and keeping the premises safe, clean, and secure through regular checks and communication. A key aspect of the role is empowering residents to take responsibility for their own lives, without doing things for them, while providing support and supervision as needed. Staff must adhere to Hestia's policies and the Ministry of Justice's guidelines.

Rough Sleeping Outreach and Assessment Worker – £34,127 per annum

Join SPEAR and help end homelessness across Richmond and Wandsworth. As a Rough Sleeping Outreach and Assessment Worker, you’ll play a vital role in our Street Outreach Service and the new 24-hour Assessment Centre. One night you could be out on the street engaging with people sleeping rough; the next day you might be welcoming someone to the centre, creating a support plan, and linking them to housing and health services.

What you’ll do

• Carry out late-night street outreach and day-time reception shifts as part of a rota.
• Build trusting relationships with people who are rough sleeping and help them move quickly into safe accommodation.
• Provide trauma-informed, person-centred support, including assessments, personalised action plans and follow-on help to sustain tenancies.

-Work closely with partner agencies to connect clients to health, housing, and other specialist services.
-Keep accurate case notes and share information securely using CHAIN and InForm systems.

About you
You bring experience of working with vulnerable adults, ideally in homelessness or a related field, and you’re confident working late-night shifts. You can build rapport quickly, stay calm under pressure, and work well with partner agencies. Above all, you believe that homelessness should be rare, brief and non-recurring.

Edward Alsop court provides support and accommodation for up to 50 men over 50's with complex needs. These needs may be around health, mobility, alcohol use, or mental health. This customer group will be catered for, with breakfast and dinner which is provided on the premises. There are also 29 self catering bed spaces for men over 35 who have low support needs and require support in order to prepare for independent living.

The service also offers specialist support to homeless men across a range of areas including re-offending behaviour, mental health issues, and substance misuse.

EAC implements and promotes the Psychologically Informed Environment (PIE) model. This is an approach to supporting people out of homelessness, in particular those who have experienced complex trauma or are diagnosed with a personality disorder. It also considers the psychological needs of staff: developing skills and knowledge, increasing motivation, job satisfaction and resilience.

You will provide information, social inclusion and vocational opportunities to support people to recover and stay well; exercise choice and control in their care and lives; and participate on an equal footing in the community. Provision of care and support is also provided to develop a model of re-enablement towards self-advocacy, self-management and empowerment, incorporating the principles and practice of recovery, co-production and peer support.

We are seeking a Wellbeing Worker to join our team in Leeds. This is a full time position which is community-based offering assessments, support, information, advice, and guidance to sensory impaired people living in Leeds.

As part of the role you will:

• Do strengths-based assessments, support plans and risk assessments with service users including and where appropriate their carers/family members.
• To work in the community which includes a person’s home when needed.
• To undertake risk assessments and report safeguarding concerns without delay in accordance with BID’s safeguarding policies and procedures.
• Promote independent living by referring people to access universal credits, personal independence payments (PIP), housing welfare. 

To be successful for this post, you will be:

• Able to communicate effectively with all people across the sensory spectrum
• Able to identify risks and conduct risk assessments to take appropriate actions
• Able to work independently, using initiative and be well organized, deal with conflicting demands on time and respond to deadline.

At the Women’s Recovery Service, you’ll be part of a dedicated accommodation service supporting women who have experienced homelessness, VAWG, poor health, substance use, and exclusion from essential services.

As our Specialist Multiple Disadvantage Worker you will bring expertise, creativity and determination to help women break down barriers and take steps forward with confidence. You will be a voice of encouragement and advocacy, ensuring women get the services and respect they deserve, and you will see the impact of your work in the real changes women make in their lives.

About the role

As an Engagement and Assessment Worker, you will be the first point of contact for someone who comes to Crisis for help to prevent or end their homelessness. You will:

• Welcome them to the service on site and at partner agencies,

• Listen carefully and non-judgementally to their story,

• Complete thorough Crisis and Mainstay assessments using a trauma informed approach,

• Agree realistic options and the support needed to assist them to prevent their homelessness or a pathway out of homelessness.

• Help them formulate a holistic plan and help them take the initial steps, where necessary.

• Hold a caseload of members who require short term intervention to end their homelessness.

About you

• Experience of working successfully with disadvantaged/or socially excluded groups and individuals, utilising a psychologically informed approach.

• Ability to deliver excellent customer service, ensuring accessible, welcoming, and safe ‘front of house’ environment in a busy service.

• Experience of assessing presenting needs, and an understanding of strengths-based assessments. You will have empathy and understanding of the barriers to accessing services.

• Ability to manage successful relationships with external agencies and relevant service providers and successfully advocate on members behalf.

• Excellent verbal and written communication skills used to ensure accurate record keeping on a variety of Case Management Systems/Databases.

• Excellent organisation and prioritisation skills, with the ability to successfully manage a busy and diverse workload.

• The ability to use your own initiative to contribute to the effective running of the service and to work flexibly as part of a team.

About the role

CTI is part of a housing-led response to ending homelessness. CTI is an evidence-based model, designed for times of transition for people who require support to navigate often fragmented systems. It uses a time limited case management approach to support people through a period of transition which works towards preventing and ending homelessness.

The model is strictly time limited and prescriptive in its application and Crisis’ CTI pilot will run close to the CTI principles. The timing of movement through the phases is defined by the Programme model.

CTI consists of four main phases:

1. Pre CTI: in which the relationship with the client is developed and their goals and ambitions are identified, (no timescale – could be days, months or years)
2. Transition: providing support & beginning to connect client to people and agencies that will assume the primary role of support and enable them to meet their goals (3 months)
3. Try out: monitoring and strengthening support network and client’s skills (3 months)
4. Transfer of care: terminating CTI and stepping right back (3 months)

The post holder will be working within the framework of CTI, supporting prison leavers from prison into accommodation, thus preventing homelessness on release from custody.

About you

To be successful in this role you will have experience of delivering person centred support with statutory and non-statutory services within the criminal justice system, offering coaching and mentoring, learning support and/or personal development training, ideally working to matrix standards. You will also have an appreciation of delivering this within a time limited framework.

You will demonstrate knowledge of issues affecting homeless or vulnerably housed individuals which could include mental illness, addiction, the benefit system and housing. Being committed to empowering homeless and vulnerably housed individuals is essential to this role.

Due to the nature of the role, it is essential that you have a valid driving licence and access to your own vehicle.
