Role overview 

Shine supports a community of over 15,000 members living with spina bifida and/or hydrocephalus, including 10,000 adults (25+).

You will be delivering high-quality support and creating opportunities for adults living with spina bifida and/or hydrocephalus and their carers, supporting Shine members to lead healthy, independent, and fulfilling lives by improving condition management and fostering connections within the Shine community. 

This role will focus on adults (25+), you will be primarily working within the Adult Services Team. However, there will be occasions when you will work across age groups to ensure the best outcomes for our members. 

The role is home-based and ideally you will live within an hour commute of London, within Essex, Kent or Sussex as you will be attending clinics in London and the Southeast as required. There would be occasional travel required across wider areas and nationally including attendance at events, conferences and meetings at our head office in Peterborough. 

The role is full time 35 hours per week, Monday - Friday with working core hours 9am-5pm with some flexibility to cover events evenings/weekends.

Key Responsibilities 

• Deliver innovative support across your region to improve health, social, and emotional outcomes for members. 

• Attend specialist clinics to engage with members and professionals. 

• Provide information, advice, and guidance to our adult members and carers, including hard-to-reach communities. 

• Support members to access benefits, services, and resources that promote financial stability, health, and independence. 

• Refer members to Shine’s Health team, internal specialists, or external agencies as appropriate. 

• Develop age-specific groups and peer support networks to reduce isolation and encourage social interaction. 

• Recruit and support Shine volunteers. 

• Contribute to regional and national events (face-to-face and virtual) based on member needs and interests. 

• Represent Shine at conferences, provide information, raise awareness among professionals, external organisations, and the public through presentations and training. 

• Promote Shine membership to individuals with relevant conditions. 

• Maintain accurate records using Shine’s CRM (Salesforce database). 

• Collaborate with colleagues across services, fundraising, marketing to identify opportunities for sustainability. 

• Maintain an up-to-date directory of contacts and share relevant information internally. 

• Undertake administrative tasks and other duties as required.

About the Role

Rough sleeping pathways only work when the system around people moves with urgency, consistency and accountability. This role is about making sure people sleeping rough, or at risk of returning to the streets, are not left waiting, drifting between services, or losing accommodation because barriers have not been gripped quickly enough.

As Outreach Housing Pathway Coordinator, you will work within Camden’s integrated Rough Sleeping and Outreach Hub to help people access and sustain appropriate accommodation. You will hold oversight of where people are in their housing journey, identify what is blocking progress, and work with outreach teams, Navigators, hostels, pathway providers, community safety partners and statutory services to keep plans moving.

You will focus on the points where pathways can break down: delays, exclusions, unsuitable offers, unresolved risks, street activity linked to accommodation, or support needs that have not yet been fully understood or responded to. Through assertive engagement, practical case coordination and strong partnership working, you will help people stabilise their accommodation and reduce the risk of returning to rough sleeping.

Working within Camden’s No Wrong Door model, you will bring grip, judgement and persistence to complex situations, ensuring people experiencing multiple disadvantage receive a response that is joined-up, trauma-informed and focused on lasting routes away from the street. SHP will support you with the training, reflective practice and development opportunities needed to continue building your expertise in this work.

About you:

• You understand that people experiencing rough sleeping are often navigating trauma, exclusion, unmet needs and systems that have not worked for them. You bring patience, persistence and a clear belief that people should not be written off when engagement is difficult.
• You know how homelessness pathways, outreach services and supported accommodation can work together, and you can spot where delays, exclusions or gaps in communication are putting someone’s route away from the street at risk.
• You are confident working with complexity. You can hold risk, need and accountability together, making balanced decisions while keeping people’s safety, dignity and long-term accommodation sustainment at the centre.
• You build relationships that help things move. Whether you are speaking with someone on the street, a hostel, a Navigator, a statutory service or a community safety partner, you communicate clearly, follow through on actions and keep people focused on solutions.
• You bring practical judgement, emotional resilience and a trauma-informed approach to your work, with the ability to stay calm and purposeful when situations are challenging, stuck or urgent.

Outreach Inclusive Recovery Worker

We're looking for a passionate and proactive Outreach Inclusive Recovery Worker to join our team on a full-time basis (35 hours per week). We are looking for a compassionate and proactive Assertive Outreach Worker to support people experiencing multiple disadvantage, including mental health needs, substance use, homelessness, social isolation, or other complex challenges. Outreach & Engagement

• Deliver assertive outreach across Surrey providing a person centred and trauma informed approach to clients referred into the project.

• Complete needs assessments and co-produce support plans tailored to individual needs.
• Monitor progress and adjust interventions accordingly.

Multi-Agency Working

• Attend multi-agency meetings (e.g., MARAC, safeguarding meetings, case conferences).
• To facilitate joint working with other support services to tackle any barriers, including Housing; Health; Social care; Domestic abuse; Drug and Alcohol and Criminal Justice.
• Advocate on behalf of clients to access services, housing, and support.

• Provide high-quality, trauma-responsive advocacy and support to women in contact with, or at risk of being in contact with the criminal justice system with a specific focus on Substance Use and Trauma.

• Involve women in the design and delivery of interventions, ensuring that they are at the heart of the work at all times.

Spectra is looking to expand our HIV support team by hiring an HIV outreach worker, who will deliver sexual health and well-being outreach and support for targeted groups in commissioned London boroughs (Brent, Ealing, Harrow, Hillingdon, Kingston, Merton Richmond, Sutton, Wandsworth), focusing on those at increased risk of poor sexual health. The postholder will deliver targeted and general HIV prevention programming, providing knowledge and skills, and empowering individuals to access the support that they need. As our HIV outreach worker, you will liaise with local service providers supporting those groups in commissioned London boroughs, supported by HIV Prevention Coordinator.

The Youth Worker (Mentoring) will support Inspiring Inclusion’s Our Schools, Our Change programme by providing trauma‑informed, one‑to‑one support to Black and racially minoritised young people with adverse childhood experiences. Working closely with Camden schools and community partners, they will build trusting relationships that help young people feel seen, heard, and empowered while wider systems change progresses. They will use holistic, strengths‑based, participatory approaches, contribute to a test‑and‑learn model, and bring strong reflective and organisational skills. Personal experience of racial profiling in state education is essential, enabling deep empathy and authenticity. The postholder will be a collaborative practitioner committed to equity, inclusion, and ensuring young people have real influence over decisions shaping their education and futures.

Part of a team of Outreach and Project Workers pivotal to the delivery of an exciting new project focused on reaching women identified as at risk of homelessness and exploitation. The Luna Project supports women at risk of rough sleeping, homelessness and exploitation. Working across key areas of Enfield and Haringey you will, through a combination of nighttime outreach and daytime service provision, be critical to supporting a coordinated response across multiple agencies already engaged in this work.

You will undertake at least twice weekly nighttime outreach, support additional services and existing outreach provision and develop a daytime offer that meets the needs of women engaged through outreach and identified as at high risk of harm and homelessness.   

You will have responsibility for a caseload of women identified through outreach, referral and presentation at ours and other services. You will provide advocacy, casework support and connection and referral into other agencies which can help them address their needs.

In addition to the around 40 young people we see every day out our centre, New Horizon provides the only youth-specific street outreach service to young people facing homelessness in London. We use our extensive housing expertise to place young people who are currently or at risk of rough sleeping into accommodation that is appropriate and safe, whilst encouraging them to access the day centre to benefit from the wide range of services we offer. This role will involve regular street outreach shifts (early mornings and late evenings) across London, aswell as supporting young people at the day centre. The role works as part of our wider Rough Sleeping team, developing a best practice approach to supporting young rough sleepers and those who are hidden homeless.

Main tasks and responsibilities

Young people services

Providing comprehensive support through 1-1 and small group work with young people sleeping rough. This will include:

1. Working within the Rough Sleeping team to ensure young people receive a high-quality service from the point of registration to move-on.

2. Carrying out at least two street-based outreach shifts each week (early morning and late evening required).

3. Carrying out housing assessments with young people and referring them into appropriate accommodation.

4. Managing a transitional case load of young people and helping them move forward and access appropriate services.

5. Working in partnership with the Outreach Team and other external teams, such as commissioned street outreach services, mental health, and substance misuse services.

6. Providing expert coaching and guidance to young people and monitoring and evaluating their progress.

7. Continued use of feedback and showing a commitment to a service that is sensitive and responsive to young people’s needs, including:

   · Completing registrations, outcomes assessments, risk assessments and action plans with young people.

   · Ensuring that all relevant work is recorded on the New Horizon database system and Chain database.

   · Ensuring risk management and assessment procedures are followed.

   Delivering advocacy and referrals to appropriate services, including making internal referrals

8. Contributing to the management and development of the team and day centre, including attending daily handovers, ensuring that Health and Safety issues are addressed at all times, attending Reflective Practice and team meetings.

9. Taking a hands-on approach to working with young people

10. Participating in the training and supervision of new staff, volunteers, students and locum workers and actively promoting a learning and development environment.

Networking and Liasion

To proactively liaise, communicate and negotiate with internal specialists and external agencies. This will include:

1. Seeking out and responding to opportunities with external partners who might offer resources, funding, or individual services relevant to young people’s needs e.g, Local Authorities, Social Services, Community Psychiatric Services, DWP, drugs and alcohol support, and other voluntary agencies.

2. Promoting and representing New Horizon Youth Centre at relevant forums and meetings.

Information management

To produce and maintain accurate and useful information in a range of formats in order to promote effective service delivery and evaluation. This will include:

1. Producing written reports in a variety of formats to meet the requirements of New Horizon, external service providers and funders, e.g., project reviews and assessment reports.

2. Inputting and extracting information from client monitoring systems other accounting and database systems. Utilising other relevant software for the production of correspondence and reports.

3. Being self-servicing in day-to-day administration and following team and NHYC’s administrative procedures.

Reviewing personal and professional development needs

To be proactive in reviewing and evaluating own performance and identifying and acting upon areas for improvement. This will include:

1. Continuously reviewing own working practices in line with feedback and current best practice.

2. Taking a proactive approach to supervision and attending clinical supervision, weekly team meetings as well as line management supervision every 4-6 weeks.

3. Reviewing and evaluating own performance to identify strengths and areas for development.

4. Undertaking development and training opportunities and being responsible for obtaining maximum benefit through review, reflection, and practice.

Other

To contribute positively and constructively to the development of the teams, service and NHYC. This will include:

1. Covering for other members of the team where necessary.
