About the Role
 

As a Senior Community Link Worker, you will have a developmental role as well as providing on-going support. You will be responsible for developing positive, working relationships between the Community Link Worker team, Primary Care and local community resources. You will work to ensure that people referred from Primary Care to the Community Link Worker service benefit from community resources which can contribute to improvement of their mental and physical health outcomes. You will contribute to community development in your local area of work by working with community organisations
 

You will carry case loads for on-going work with specific individuals for developing and reviewing personal support plans, and provide oversight of personal support plans designed and implemented by Community Link Workers. Working closely with Community Link Workers in a guidance and/or support and supervision role, you will promote good working practice and staff development.
 

You will also ensure that the Community Link Worker service provides an environment where people can improve their health outcomes, establish improved connections in their communities and enhance their quality of life.
 

Key Responsibilities:

• Work autonomously being able to judge when I refer to your manager and take a lead role in developing the service. 
• Working closely with the Project Manager and Locality Manager to achieve strategic outcomes
• Working closely with colleagues in Primary Care
• Working to develop improved, working links between Primary Care and local community resources
• Identify gaps in the service where they arise and implement solutions in partnership with your line manager
• Linking service users to appropriate resources in the local community
• Compiling and reviewing service user personal support plans ensuring outcomes are being achieved, including ensuring people are connected to their community
• Oversight of personal support plans developed and implemented by Community Link Workers
• Overseeing Community Link Worker workloads 
• Ensuring continuity of service within the context of change MH values and strategic outcomes
• Ensuring that careful and ongoing review systems are in place and maintained
• Building positive working relationships with Primary Care and other local partners to better support our service users and to promote the work of the Community Link Worker service
• Collating information and prepare agency reports for line management as required
• Contributing to Research and Development at a local level in support of National objectives to promote service improvements
• Travelling throughout the geographic region covered by the service as required

Essential Criteria:

• Educated to SVQ 3 level or have experience working in the social care sector.
• Work well in a team setting but can also work autonomously 
• Have a sound theory base of social care interventions including risk assessment 
• Have a value base consistent with the aims and objectives of Change Mental Health
• Able to apply an analytical approach to problems in order to find solutions
• Competent in planning and reviewing development plans and taking appropriate action if needed
• Good Standard of I.T skills
• An understanding of the importance of providing professional support and supervision to practitioners
• Sound knowledge of Health and Social care policy in Scotland and can apply this to my role
• Desire to develop my knowledge and skills and attend training as required
• Ability to travel throughout the geographic region covered by the service as required

Desirable Criteria

• Experience of working with people affected by mental or physical health issues
• Experience of community-development work
• Experience of working in a SSSC Registered Service
• A learning and development portfolio which evidences your skills and knowledge

General Duties:

• Act in accordance with Data Protection legislation.  Ensure all records, personal, staff and client data are managed in line with Data Management and Information Governance policies
• Comply with legal and regulatory requirements such as provisions set out in the Health and Safety at Work Act 1974
• As with all employees, workers and volunteers; to encourage people to join Support in Mind Scotland as a member, donor or activist

Do you want to develop and facilitate a new youth development programme at Share? We are looking for a passionate and creative team member to join as our new Youth Development Facilitator. 

Share is a registered charity and a centre for training and wellbeing. You’ll help people build the life skills they need to make positive long-term changes. You’ll develop and facilitate our new programme at Share, delivering session both at our training centres and out in the community. You’ll help our students achieve their personal objectives and keep learning in an engaging, fun and stimulating way.

• You’ll think creatively when planning and facilitating the programme and use a range of methods to support the students learning goal

• You’ll support progression pathways for young people with a learning disability and/or are autistic, many of whom have recently left college

• You’ll work with each young person to understand their individuals' preferences and strengths to develop an accessible action plan

• You’ll work to strengthen our local community and youth networks, developing partnerships with external organisations

About the role 

This role delivers the Pan-London Sexual Violence Alliance (PLSVA) prevention and outreach priorities, strengthening community and professional responses to sexual violence across London.

You will design and deliver trauma-informed, culturally responsive prevention work, including campaigns, training and community engagement, with a focus on Black and racialised communities, LGBTQ+ people, men and boys, and people with learning disabilities. Working within an intersectional feminist framework, you will promote accountability, collective action and clear survivor pathways.

As the Community Prevention Worker, you will:

• Design and deliver inclusive community prevention programmes, awareness campaigns and training on sexual violence, consent, bystander intervention and male accountability.
• Build and maintain strong partnerships with communities, grassroots organisations and statutory and voluntary sector agencies to improve awareness, responses and referral pathways.
• Work closely with Alliance Navigators, partner agencies and the other Community Prevention Worker to ensure consistent messaging, effective signposting and joined-up survivor pathways.
• Embed survivor voice and co-production through collaboration with the Fearless Collective (Experts by Experience).
• Monitor, evaluate and report on prevention and engagement activity, contributing to Alliance learning, impact reporting and service improvement.
• Model WGN’s feminist, anti-oppressive, trauma-informed practice and operate within safeguarding and good practice standards.

Responsibilities

About the role: 

• Under the supervision of CGL’s Cluster Manager, you will lead the delivery of services that empower Children, Young People, and their Families to overcome issues around substance misuse.
• The post holder will provide line management to practitioners within the service and will be responsible for the operational management and strategic development of the service.
• They will always maintain high professional standards and ensure a Young Person focused approach in all aspects of delivery with the aim of reducing harm and promoting resilience.
• They will lead by example in creating a professional and young persons focused team culture focused on excellent service delivery.
• The post holder will ensure a positive learning environment where integrity and professionalism are paramount, whilst empowering staff to ensure all attempts at positive outcomes are achieved for those using the service.
• You will support the Cluster Manager in the continual development and improvement of service delivery.
• You will develop and maintain excellent working relationships with partners both internally within the service and externally within a wider partnership.
• The Post Holder must have a valid UK Driving License and access to a car in order to work flexibly at various operational sites as required.

About you: 

• Experience of working with children and young people experiencing difficulties around Substance misuse.
• Knowledge of the issues facing children, young people and their families affected by substance misuse.
• Excellent knowledge of drugs and alcohol and their effects.
• Line management experience including the delivery of reflective practice.
• The ability to lead, empower and motivate a team.
• Excellent knowledge of safeguarding policies and practices relating to children and young people and experience of embedding these operationally.
• The ability to foster new partnerships and to maintain positive relationships with partner agencies.
• Knowledge of a range of psychosocial and evidence-based interventions around substance misuse.
• Knowledge and understanding around the ‘Hidden Harm’ agenda.
• Knowledge and experience of community-based support/outreach practices.
• Ability to ensure all record keeping, outcome monitoring and data collection is maintained effectively.
• Communicate confidently and effectively, verbally and in writing.
• Respond flexibly to the demands of the post.
• The ability to keep calm under pressure.
• Show commitment to facilitating positive outcomes for young people and their families.

Keychange is a Christian charity with over 100 years of experience supporting people facing vulnerability. Today, we provide specialist housing for women and young people experiencing homelessness, alongside residential care for older people across the South and South West of England.

We are now seeking an exceptional Women’s Homelessness Lead (Surrey) to shape and lead our specialist women’s homelessness service at Wayside Community in Reigate. This community is a 19 bed, 24-7 supported housing for women experiencing homelessness. This is a rare opportunity to combine frontline leadership, strategic development, and church and community partnership building in a role with real depth, influence and impact.

About the Role

• This is a leadership role with both operational and strategic responsibility. You will:
• Provide leadership and line management to the Deputy Manager, Senior Administrator and a skilled team of support workers
• Lead the delivery of trauma-informed, strengths-based, person-centred support
• Develop strong partnership networks across local authorities, charities, housing providers and churches
• Shape the future of Keychange’s women’s homelessness strategy across Surrey

About the Peer Support Services

Our Peer Support Service started 14 years ago, Since then, we have seen our service grow and we now offer Peer Support across the whole of Hertfordshire.

Working in co-production with clients, staff, volunteers and stakeholders, we were able to create our own Peer Support Charter Values (this can be found on our website).  These values form the ethos we work towards within our peer support team.  These values also complement HMN’s values of Hope, Courage, Togetherness and Responsive, which form the basis of all the work we do as an organisation.

About the Role

Working with the team to offer 1-1 peer support to anyone accessing HMN. The Peer Support service provide tailored support for individuals to support their needs. This includes using different tools and techniques to help an individual improve areas of their life they may be struggling with. Having a lived experience of mental health issues or supporting someone who has is a key element to what makes this service so unique.

The Peer Support approach includes empathy, warmth, acceptance, authenticity and compassion. Peer Support is based on the recognition that there is no better person to support the path towards recovery, than someone who has walked a similar path as that person.

To meet client needs, and work to this personalised approach, we do support people out in the community, within their home, as well as in one of our Wellbeing Centres.

The successful candidate will need a personal understanding of mental ill health, either from your own lived experience or by caring for someone with a mental health condition and have awareness of issues in mental health service provision.

Peer Support Workers will also provide support within our Peer Support Groups, Peer Support Learning Sets and Peer Support Academy when needed.

We offer:

• Annual leave entitlement of 25 days per year pro rata, rising in increments to a max. of 28 days after 5 years employment (plus 8 days Bank Holidays)
