Welfare and Dementia Activities Officer

Location: Based at Head Office, with delivery of dementia activities in St Helens area and occasional welfare visits to Scotland and the South of England

Job Purpose

To provide remote welfare support to beneficiaries, with occasional visits across Scotland and Southern England, helping them access statutory and voluntary services, maximise their income, and receive practical advice and guidance.

Lead the planning, coordination, and delivery of dementia activities in St Helens, creating opportunities for beneficiaries and their families to connect, participate and improve their overall wellbeing.

Key Responsibilities

• Provide remote welfare support to beneficiaries across the UK, assessing their needs and providing appropriate advice, guidance, and signposting.
• Plan and undertake occasional beneficiary visits as part of the Trust’s visiting programme across the UK, where appropriate to the individual’s needs.
• Support beneficiaries to access statutory and voluntary services, benefits, grants, and other sources of financial or practical support.
• Make and follow up internal and external referrals as required.
• Identify and escalate welfare, safeguarding and urgent concerns to the relevant authorities, keeping the Head of Welfare and Communities informed.
• Maintain accurate and up-to-date records of beneficiary contacts, assessments, referrals, and outcomes.
• Lead the planning, organisation, and delivery of dementia activities across St Helens, including lunches, day trips, carer support, training sessions and social activities, working collaboratively with our Admiral Nurse and Welfare colleagues.
• Recruit, support and coordinate volunteers to assist with activity delivery and events.
• Develop and maintain local networks with relevant organisations and professionals to identify opportunities for joint initiatives and collaborative approaches to welfare and dementia support and activities.
• Promote relevant Trust services, including Ruskin Lodge Respite Care Home, and support referrals where appropriate.
• Represent the Trust at relevant meetings, events and community activities.
• Produce a monthly report on activity and outcomes.
• Participate in relevant training and professional development.
• Undertake other duties appropriate to the role, commensurate with the post holder’s skills and experience.

It is important to live in or close to Central Lancashire – eg. South Ribble, Preston, Chorley, and surrounds.

This is a community-based role with a special focus on telephone support, specialising in being the first voice and support to reach people in need of our help. You will be speaking with people from across Lancashire and South Cumbria many of whom may have waited months for a diagnosis, for answers and progress, or been living with new symptoms or challenges for some time before asking for help.

Alongside working from home, the role will also involve regular travel across primarily the Central Lancashire area including local outreach, meetings, direct support, and group activity provisions based on business and community needs.

You will work closely with people living with dementia and those who care about them, providing ongoing support. You will listen carefully, understand what matters most to each person and offer guidance that helps them feel steadier and more confident as their situation changes.
Your days will be varied and meaningful. You might be offering calm reassurance over the phone, or working closely with other professionals to make sure the right support is in place. You might be the Dementia Adviser on hand at a Singing for the Brain Group, or representing Alzheimer’s Society by visiting a local partner’s activity or an event on outreach.

We operate a triage system for new referrals and you will play a key part in making sure people are responded to quickly and thoughtfully. You will manage your own caseload, keeping clear and accurate records, while always seeing the person behind the paperwork.

You will be part of a caring and dynamic local team who share ideas, reflect together and support one another. There will be time to meet as a team and opportunities to keep growing your knowledge of dementia, with your manager and colleagues alongside you. We provide training and ongoing support.

Our Hospital Dementia Adviser provides vital support to people living with dementia during their hospital stay. This is a hospital-based role that involves regular walking between wards and departments. You will work closely with the wider Local Services Team to ensure that people are smoothly connected to ongoing support in the community once they leave the hospital.

£37,500 per year, 35 hours a week Mon-Fri, permanent, based in SE22 Southwark London.

Minimum 3 days a week office/community based.

As Service Coordinator, you will play a key role in ensuring older people access high-quality, person-centred support. You will coordinate and deliver services, build relationships with partners, referrers and volunteers, and help shape new projects that improve wellbeing and reduce isolation across the community.

Responsibilities include:

· Coordinating our Information and Support service.

· Line managing the Information and Support Facilitators.

· Being the operational point of contact for the Ageing Well Southwark contract.

· Managing and developing projects.

· Coordinating befriending, groups and social events.

Age UK Croydon is looking for an enthusiastic and motivated Exercise and Wellbeing Officer to help guide our Healthy Lifestyles Service. Our Healthy Lifestyles Service delivers a range of activities including seated and standing exercise programmes, health checks, social engagement opportunities, health hubs and Memory Tree Cafés for people living with dementia and their carers. Services are delivered across community venues and care homes throughout Croydon.

About the Role

As a Exercise and Wellbeing Officer, you will play a pivotal role in improving the health, wellbeing and independence of older people across the community. Working with the Healthy Lifestyles Service Manager, you will help shape and deliver a range of meaningful activities while providing day-to-day support and guidance to staff and volunteers.

Key responsibilities include (but are not limited to):

· Prepare and deliver seated and standing physical activity sessions in care homes and community venues across Croydon.

· Carrying out basic health checks, including blood pressure and other wellbeing measurements.

· Supporting the delivery of Health Hubs, wellbeing events and health promotion activities.

· Building positive relationships with care homes, community venues, partner organisations and guest speakers.

This is a varied and rewarding role, offering the opportunity to combine leadership, partnership working and direct service delivery while helping older people stay active, connected and independent.

About You

We are looking for someone who is energetic, approachable and committed to supporting older people to live healthier,

more connected lives.

You will have:

· Level 2 Fitness Instructor qualification, Exercise Referral qualification, Otago Exercise Programme, REACT qualification, or another relevant physical activity, fitness, falls prevention or rehabilitation qualification.

· Good understanding of the issues affecting the health and well-being of older people including exercise, healthy eating, nutrition and malnutrition

· Good understanding of the needs of older people including dementia

· Excellent working knowledge of administration processes and systems

· Good knowledge of IT systems including Excel, PowerPoint and Word

· Experience of compiling monitoring reports, reporting on quality, performance KPIs, project outcomes and impact

Age UK Bromley & Greenwich is seeking a dedicated and compassionate Care Navigator to join our expanding team in partnership with Oxleas NHS Trust. Based at Memorial Hospital in Woolwich, you will be embedded within a multi-disciplinary team working on the Proactive and Frailty pathways. Your focus will be on supporting older people with complex needs, helping them navigate health and social care systems, and connecting them to relevant services across the community.

This is a key role that sits at the heart of person-centred care. You will attend multi-disciplinary team meetings, work alongside professionals including GPs, case managers and mental health staff, and spend time speaking directly with patients and their families. Your role will be to truly listen, identify what matters most to the individual, and help them take practical steps towards better health, independence and wellbeing.

Key responsibilities include:

• Attending Multi-Disciplinary Team (MDT) meetings and contributing to holistic care planning

• Working with patients to understand their needs, provide emotional support and signpost to local services

• Liaising with a wide range of professionals across the NHS, social care and voluntary sectors

• Inputting information into systems including RIO and Framework I, with training provided

• Keeping accurate records and contributing to service evaluation and improvement

Senior Exercise and Wellbeing Officer

Age UK Croydon is looking for an enthusiastic and motivated Senior Exercise and Wellbeing Officer to help guide our Healthy Lifestyles Service. Our Healthy Lifestyles Service delivers a range of activities including seated and standing exercise programmes, health checks, social engagement opportunities, health hubs and Memory Tree Cafés for people living with dementia and their carers. Services are delivered across community venues and care homes throughout Croydon.

About the Role

As our Senior Exercise and Wellbeing Officer, you will play a pivotal role in improving the health, wellbeing and independence of older people across the community. Working closely with the Healthy Lifestyles Service Manager, you will help shape and deliver a range of meaningful activities while providing day-to-day support and guidance to staff and volunteers.

Key responsibilities include (but are not limited to):

• Leading, supporting and motivating two Exercise and Wellbeing Officer and a dedicated team of volunteers.

• Delivering engaging seated and standing exercise sessions that help participants improve their mobility, strength, balance and confidence.

• Building and nurturing strong relationships with care homes, community venues, health professionals and partner organisations to maximise our reach and impact.

• Supporting the planning, development and continuous improvement of health and wellbeing programmes that make a real difference to people's lives.

This is a varied and rewarding role, offering the opportunity to combine leadership, partnership working and direct service delivery while helping older people stay active, connected and independent.

About You

We are looking for someone who is energetic, approachable and committed to supporting older people to live healthier, more connected lives. 

You will have:

• Level 2 Fitness Instructor qualification, Exercise Referral qualification, Otago Exercise Programme, REACT qualification, or another relevant physical activity, fitness, falls prevention or rehabilitation qualification.

• Experience of monitoring projects and reporting on outcomes and performance.

• Knowledge of the health and social care sector

• Strong communication, organisational and administrative skills

• Good IT skills, including Microsoft Office and database systems.

• Experience or qualifications in health promotion, nutrition, health checks, chair-based exercise or group exercise delivery.

• Experience of risk assessment and health and safety management.

• Specialist Casework & Income Maximisation: Deliver 1-to-1 welfare benefits casework, carry out calculations, complete application forms, and support carers through mandatory reconsiderations and appeals (covering PIP, Carer’s Allowance, Universal Credit, Attendance Allowance, etc.).

• Workshops & Outreach: Design and facilitate engaging information workshops and drop-in sessions (online and face-to-face) on key benefit topics affecting carers.

• Collaborative Support: Respond to internal and external referrals (from health, social care, and voluntary partners), working closely with team leaders to support particularly vulnerable carers.

• Partnership & Impact: Maintain accurate case records, track outcome metrics (e.g., financial gains), and represent Carers' Hub with local statutory and voluntary services.

The successful candidate will shape the strategic vision for more than 1,800 families, oversee the close partnership between the Society and its research subsidiary RDRP, and champion the patient and family ethos that sits at the heart of the organisation's work. As our next Chief Executive, you will

Salary to be agreed depending on experience and will be pro-rata for part time hours.

This is an essential car user post. The applicant must hold a current UK driver’s licence, with no more than 6 points, have access to a car and be able and willing to drive UK wide as required.

This senior role requires flexibility in working hours and location.  It is primarily offered on a hybrid basis with a minimum of 2 days in our Amersham, Buckinghamshire, office each week. As part of the role, UK and overseas travel is necessary, which may on occasion include early morning and/or evening working and sometimes overnight stays. We have policies in place to ensure that any unsociable hours worked over the weekend are fairly compensated through TOIL.
