As Homeless Pathway Coordinator, you’ll be a consistent and trusted presence for patients experiencing homelessness, helping them navigate what can feel like an overwhelming mix of health, housing and support systems. You’ll spend time with patients on wards and in A&E, listening to what matters to them, understanding the barriers they face and making sure their voice remains at the heart of every decision.
Day to day, you’ll manage a caseload, complete assessments, develop clear discharge and support plans and make referrals to housing, healthcare, social care, mental health and substance use services. You’ll follow up actions, keep plans moving and advocate firmly when someone is at risk of being discharged without the right accommodation or support in place.
You’ll work closely with the Homeless Pathway Lead, hospital teams, Newham Council and SHP’s wider Health Team, bringing professionals together and helping everyone stay focused on the person behind the case. You’ll also build links with community services, identify gaps in existing pathways and share learning that helps the hospital respond more effectively to people experiencing homelessness.
This is a role where strong relationships can change the course of someone’s recovery. Carers often experience significant financial challenges because of their caring role. This post will play a key role in ensuring carers can access the information, advice and support they need to maximise their income, improve financial resilience, and make informed decisions about their finances.
The Carer Financial Wellbeing Coordinator will provide day-to-day line management and support to Welfare Benefits Advisors, oversee the quality of financial wellbeing services, and work collaboratively to develop innovative and accessible support for carers.
The role includes providing welfare benefits and financial wellbeing advice, supporting complex cases where appropriate, delivering workshops and training, developing resources, and identifying opportunities to strengthen our support offer.
You will work closely with teams across Carers Support West Sussex to ensure carers receive joined-up support and that financial wellbeing is embedded throughout our services. This role will predominantly support the Service Delivery Managers and Support Officer across the UK in the effective planning and delivery of wheelchair skills training to meet the strategic objectives. The postholder will be required to work directly with young people and their families, schools, colleagues, partner organisations and professionals, delivering accredited training across the UK.

You will be part of the Young People’s Services Team who provide a range of services and activities for young wheelchair users and their families. You will have a friendly and professional approach to working with families and professionals and be highly organised to manage planning, delivery and relevant paperwork for all wheelchair skills services.

About the role
As a Senior Practitioner, you will manage a complex caseload, providing independent, rights-based advocacy and one-to-one support to care-experienced young mothers. Working autonomously while supported by the Project Coordinator, you will empower young mothers to understand their rights, navigate statutory processes and make informed decisions that help keep families safely together.
You will build trust, complete assessments, develop support plans and advocate on their behalf within multi-agency settings. You will work alongside social workers, midwives, health visitors and other professionals to ensure each young mother's voice is heard and represented.
Alongside direct casework, you will contribute to peer support groups, workshops and community-based activities that reduce isolation, build confidence and strengthen parenting skills. You will also play an important role in safeguarding, quality assurance and service development by maintaining high-quality records, producing case studies and identifying opportunities to improve local services.

About you
We are looking for someone who is passionate about supporting vulnerable young people and families. You will be confident working independently while building strong partnerships with professionals across health, social care and the voluntary sector.
You will have experience supporting families or parents with complex social care involvement and a sound understanding of safeguarding, risk assessment and relevant legislation. You will be an effective advocate with excellent communication skills, able to build trusting relationships while maintaining professional boundaries.
You will be organised, resilient and confident managing a demanding caseload, with strong analytical skills and the ability to make sound professional judgements in complex situations.
