This peer support project is delivered in partnership with South West London and St George’s Mental Health NHS Trust to support Carers of people with mental health conditions. This project will work to support Carers through one-to-one support, outreach, training and peer support groups.
About The Role:
Using your lived experience of caring for someone with a mental health condition, and a strengths-based approach, you will support mental health Carers in their caring role. You will provide person centred one-to-one- support, information, signposting, and advocacy, develop, and facilitate peer support groups and deliver training and outreach sessions at different locations across the borough.

Young Carers Support and Development Worker
Salary: £31,402.80 per annum (inc. Inner London Weighting)
Full time (35 hours per week)
The role:
You will work across three key age groups, 5–11, 12–16 and 16-25, delivering and supervising activities, supporting events and trips, and providing behavioural management where needed as well as being required to plan and facilitate a weekly After School club for ages 5-11 and 12-16. Alongside group activities, you will offer one-to-one support to individual Young Carers and Young Adult Carers, ensuring they feel heard, included, and supported as they navigate the challenges of their caring roles. You will also deliver awareness training sessions, run stalls, and build partnerships in the community to identify and engage Young Carers.
Skills and experience:
The successful applicant will have outstanding organisational and communication skills, be confident organising trips and activities for children and experience working with children. About the role:
As a Project Worker at our Stacey Street service in Islington, you’ll support adults who have experienced homelessness or rough sleeping to rebuild stability and move towards independent living. This is a hands-on role in a busy hostel environment where no two days are the same.
You’ll work directly with residents who may be navigating challenges linked to mental health, substance use, trauma or contact with the criminal justice system. Through strengths-based support planning and clear risk management, you’ll help individuals identify goals, overcome barriers and take practical steps forward. From supporting someone to engage with treatment, to sustaining a tenancy or accessing training and employment, your work will focus on realistic, sustainable progress.
Day to day, you’ll build trusted relationships whilst maintaining professional boundaries. You’ll keep accurate records, respond calmly in challenging situations and contribute to a psychologically informed environment where people feel respected and motivated to change.

About the role:
Real change rarely happens in a straight line. At our Milton House project, you’ll work alongside people who have experienced homelessness, trauma and multiple disadvantage, offering the consistency, practical support and belief that can help them begin moving forward.
As a Project Worker, you’ll hold a caseload and build purposeful, trusting relationships with clients. You’ll complete assessments, develop person-centred support and safety plans, and help people identify goals that feel meaningful and achievable to them. This could include supporting someone to improve their health, address substance use, manage their accommodation, strengthen their daily living skills or reconnect with services and opportunities in their community.
You’ll be there when situations become challenging, responding calmly to risk, safeguarding concerns and periods of crisis while continuing to recognise each person’s strengths and potential. Working closely with colleagues and external partners, you’ll advocate for clients, coordinate joined-up support and help remove the barriers that have previously prevented them from accessing the help they need.
This is a hands-on and varied role where no two days will look the same. Alongside direct support, you’ll contribute to the safe and effective running of the accommodation, maintain clear and accurate records and help create an environment where clients are treated with dignity, respect and hope. You’ll also receive ongoing training, reflective practice and development opportunities to strengthen your skills and build your future career within SHP.
The working rota for this post is Monday - Friday, with no weekend shifts. The rota will consist of a mixture of earlies (8am - 3.30pm) & lates (3pm - 10.30pm).

We’re looking for a compassionate and motivated Community Connector to join our GP Integrated Mental Health Service and Mental Health Integrated Community Service in Surrey Heath.

About the role

We are looking for a compassionate and motivated Assertive Outreach Worker to join our Liaison and Diversion Service. Working alongside criminal justice, health and community partners, you will support vulnerable people with mental health needs, learning disabilities, substance misuse issues and other complex challenges to access the services they need. Through person-centred outreach and practical support, you will help individuals improve their wellbeing, reduce barriers to engagement and achieve positive outcomes at a crucial point in their lives.
Catalyst Support, in partnership with Surrey and Borders Partnership NHS Foundation Trust, provides the Liaison and Diversion Service across Surrey. The service aims to identify and support vulnerable people of all ages at the earliest opportunity when they come into contact with the criminal justice system. As an Assertive Outreach Worker, you will provide person-centred, community-based support to individuals following assessment by the Liaison and Diversion team. The Counselling Outreach Worker is key to SEL Mind's exciting new counselling service for Black residents of Southwark, delivered as part of the wider Southwark Wellbeing Hub. This part-time role will shape the Wellbeing Hub's provision to Black communities, expanding equitable access to culturally-appropriate support.

About the Service
The Complex Needs Housing Service works as a multi-disciplinary team with housing teams and Drug and Alcohol Services across District and Borough councils. These teams will be working with individuals who are presenting as homeless and have a number of Complex Needs. The service has been running since 2022 with a dual-district model, however, is now expanding to offer a single district outreach service.
We have roles available in the service in the following areas:
- St. Albans – supporting individuals who are homeless, in temporary accommodation or hostels in the district with their mental health
- South & West Hertfordshire – supporting women who have fled domestic abuse and currently residing in refuge/safe accommodation in the area with their mental health N.B. Only female applicants will be considered for this role.
The objectives of the Single Homeless Support Pathway are:

About the Project
Herts Mind Network (HMN) has a partnership with Hertfordshire Partnership Foundation Trust (HPFT) to deliver a one-to-one outreach service for people who are in hospitals across Hertfordshire.
There is an emergency department arm of the project and will be based at Watford General Hospital working in A&E departments, with the Mental Health Liaison Team, and hospital wards facilitating hospital discharge for people with mental health need.
About the Role:
The Bounce Back Worker will be based at Watford General Hospital and HMN Centres, promoting joint working between Watford A&E, creating a seamless pathway into community support for clients recovering from mental ill health.
The aims of the Bounce Back service are to provide up to 16 sessions of emotional and practical support and advice & information to patients. We will visit patients on the ward and continue the sessions in the local community and in patient’s homes once discharged. We will facilitate a smooth and timely hospital discharge.
Key Responsibilities
The Bounce Back worker will use a recovery orientated approach working with patients on the ward, and in the local community, including in people’s homes. The recovery approach includes empathy, warmth, acceptance, authenticity, compassion and humanity. To work with hospital staff to ensure patients are offered advice, information, and holistic support.
The post holder will need to:
- Provide practical and emotional support to encourage patients to develop their independence within their local community.
- Facilitate a smooth discharge from hospital back to the community.
- Have a solid understanding on the dual effect of mental health and drugs and alcohol.

You will provide advice, practical support and befriending to a caseload of participants, aged 11-30, who are at risk of/involved in offending; to encourage and enable them to participate in Carney’s Community’s personal development opportunities and to access the wider services and support they may need in order to give them the best chances of being “the best they can be”. This will involve supporting young people to engage in positive and constructive activities, attend Youth Justice Service or Probation appointments and creating opportunities for their personal, social and emotional development
You will work alongside the employment support manager to plan and deliver the employment group, supporting participants that attend this group and leading specific employment programmes. As manager of the key work project you will oversee the small team of key workers, provide support, guidance and supervision. You will be involved in monitoring and reporting for the project, collecting feedback from participants. You will manage referrals to the project from other organisations including schools, social services, probation and the youth justice service.
About You:
You must have proven experience of working directly with young people in a youth work or key work setting, providing one to one support. You will have the ability to empathise and mediate for young people, a strong understanding of the local area, barriers facing young people, the interplay between material disadvantage, vulnerability and social exclusion and be highly skilled in diplomacy, professional distance, demonstrating empathy, boundaries and sound judgement. You will have experience managing other staff or volunteers.

Carney’s works differently by providing long-term, consistent, unconditional support with empathy; a safe space and activities including boxing, bike workshops, employment support, studio sessions, mentoring and key work. Inspired by boxing legend Mick Carney, the charity was founded in his name after his passing in 2011, building on work led by our co-founders. We aim to create a “virtuous cycle”: while support is free of charge, we ask that when young people are ready, they channel the positivity they have received into others. As a result, many go on to volunteer, mentor, join our staff team or even set up their own community initiatives. Former participants make up the majority of our workforce. They provide the most authentic role models, people who genuinely understand the challenges our young people face and who show what is possible.
