We are seeking someone with proven experience supporting individuals with mental health and/or dual diagnosis, with strong knowledge of mental health conditions, recovery approaches, and relevant legislation. The role requires excellent communication, partnership-building, and case management skills, alongside the ability to work independently, as part of a team, and to support new staff or volunteers. You should have experience in planning and delivering initiatives such as groups, events, or volunteer programmes, as well as strong organisational, IT, and administrative skills. A solid understanding of safeguarding, risk assessment, and local recovery services is essential, along with the confidence to represent the organisation externally and manage work effectively under pressure.

When will I be working?

39 hours per week, including weekends and bank holidays. The service is open 365 days a year from 2pm - 10pm

Our Service

NYAS provides mental health advocacy to individuals within mental health settings throughout the UK.

Our service provides independent advocacy to people within mental health hospitals and community-based settings. We support individuals to ensure their voice is heard regarding their care and treatment and that they are aware of their rights.

This also includes representing them during various meetings such as ward rounds and at multi-disciplinary team meetings.  This role requires you to produce written reports to ensure that the views of patients are fully represented.

We work with adults and children and young people.

DO YOU HAVE:

• Direct experience of working with children/young people or vulnerable adults in a social care or mental health setting.
• Experience and understanding of child protection/vulnerable adult safeguarding procedures
• Ability to maintain professionalism at all times

Young People Support Worker

We are seeking a compassionate Young People Support Worker to help young people build the confidence and skills needed for independent living.

About the role

Join a close-knit and supportive team working tirelessly to ensure some of the borough’s most vulnerable young people receive the care and support they need.

As Young People Support Worker, you will support young people aged 18 to 25 living within a six-bed, low-support accommodation service. Working every Saturday and Sunday, you will help residents develop their confidence, resilience and independent living skills.

Your key responsibilities will include:

· Managing a caseload and acting as the named key worker for individual residents

· Completing risk and needs assessments and developing SMART, outcome-focused support plans

· Delivering one-to-one and group support within the accommodation and local community

· Supporting residents to access education, training, employment and volunteering opportunities

· Helping residents maximise their benefit entitlement, including Housing Benefit and Universal Credit claims

· Working with statutory services and other agencies to provide joined-up support

· Welcoming new residents and helping them settle into the service

· Maintaining a safe environment through safeguarding, health and safety checks and appropriate reporting

· Encouraging residents to influence decisions about their support and participate in wider activities

The role involves two 7.5-hour shifts each weekend, with working hours arranged flexibly between 8am and 6pm on Saturdays and Sundays.

You will support people experiencing a range of complex challenges, including severe mental health difficulties, self-harm, suicidal thoughts, psychotic experiences and substance misuse, using de-escalation and other appropriate non-clinical interventions. Active Essex Foundation is a successful and growing charity dedicated to creating the conditions for young people and communities across Essex to thrive. Our work focuses on three core areas: improving young people’s mental health, reducing vulnerabilities to youth crime and exploitation, and strengthening communities through trusted relationships and positive engagement.

A defining feature of our approach is our network of more than 100 Locally Trusted Organisations (LTOs) across Essex. The Sport for Development Officer will play a key role in supporting the delivery and growth of these programmes. Working closely with our programme leads, locally trusted organisations (LTOs), and community stakeholders, the postholder will help strengthen organisational capability, and ensure high-quality delivery across our mental health, youth crime prevention and community strengthening work. The role will also contribute to the leadership of workstreams aligned with our strategic Outcomes, supporting the Foundation’s mission.

Job Purpose:

To support the delivery of Oasis Community Housing’s Young People’s Supported Housing Service (Ofsted registered). The service operates two 24-hour staffed supported housing projects, one for children and young people aged 16-17 with medium needs, and one for those aged 16-21 with complex needs. To provide trauma informed, holistic, personalised support to children and young people, which is in line with relevant pathway and safety plans. 

To work closely with Gateshead Council, including Children’s Services, to ensure children and young people achieve the best outcomes, in line with project specific aims and objectives. Under the guidance of the Team Leader and Registered Service Manager, to ensure the service is compliant with the Supported Accommodation Regulations and we maintain successful Ofsted registration for these projects.

Our projects are places of safety, Psychologically Informed Environments (PIE), from which we work with children and young people using person-led, holistic, trauma-informed and solution-focused approaches. We aim to provide accommodation and support that helps children and young people find self-worth and the belief that they deserve a better future, supporting them in safe, positive transitions including where relevant from care into more independent accommodation, and from childhood to adulthood; providing holistic support to gain essential life skills, achieve health outcomes, to realise and achieve their aspirations, and reach their full potential. 

This role is based in a new service, which goes live on 1st June 2025:

• A 24-7 staffed 8 bed Ofsted registered supported housing project for young people aged 16-21 with complex needs.

• A 24-7 staffed 5 bed Ofsted registered supported housing project for young people aged 16-17 with medium needs.

Duties & Responsibilities:

• To provide holistic (mental, physical, emotional, spiritual) support to children and young people within our supported housing projects, in accordance with the ethos and values of Oasis Community Housing. Ensuring support provision is personalised to individual needs, and progress is regularly reviewed.

• To engage in multi-disciplinary team (MDT) approaches to support children and young people, including working closely with Children’s Services and other professionals and partner agencies. To include; attending meetings and effective information sharing.

• To ensure safeguarding policies and procedures are adhered to in your practices, and that concerns are reported appropriately and expediently, both internally, and externally, as needed.

• To navigate challenging situations effectively and sensitively.

• To maintain records needed to ensure excellent and consistent support provision; maintaining comprehensive case files, using our In-Form data system, completing reports and assisting in the measurement of outcomes.

This role is responsible for providing day-to-day leadership to a team of Recovery Workers, ensuring the safe, effective and smooth operation of the Recovery House. The postholder will work collaboratively with NHS crisis services, statutory and voluntary sector partners, monitor performance and compliance, respond to incidents and safeguarding concerns, and support continuous service improvement. Job Purpose:

To support the delivery of Oasis Community Housing’s Young People’s Supported Housing Service (including Ofsted registered project). The service supports 56 young people at any given time; in one 16-bed Ofsted registered 24-hours staffed supported housing project for 16-21 year olds with medium-high needs, plus visiting support to young people living in properties across the North of the Borough. 

To provide trauma informed, holistic, personalised support to children and young people, which is in line with relevant pathway and safety plans. To work closely with Southwark Council, including Children’s Services, to ensure children and young people achieve the best outcomes, in line with project specific aims and objectives. Under the guidance of the Team Leader and Registered Service Manager, to ensure the service is compliant with the Supported Accommodation Regulations and we maintain successful Ofsted registration for the relevant project.

Our projects are places of safety, Psychologically Informed Environments (PIE), from which we work with children and young people using person-led, holistic, trauma-informed and solution-focused approaches. We aim to provide accommodation and support that helps children and young people find self-worth and the belief that they deserve a better future, supporting them in safe, positive transitions including where relevant from care into more independent accommodation, and from childhood to adulthood; providing holistic support to gain essential life skills, achieve health outcomes, to realise and achieve their aspirations, and reach their full potential.

We are commissioned by Southwark Council to provide this service, which currently consists of:

• A 24-7 staffed 16 bed Ofsted registered supported housing project for young people aged 16-21.

• A 4 bed property for young mums and their babies/toddler age children - with a staff office, visiting staff support Mon-Fri 9-5, and on call support outside of working hours.

Role Context

Recovery in Community is a collaborative initiative delivered in partnership between Gaddum and Manchester Mind. The service supports people to leave inpatient mental health settings and return to their community with the support they need to recover and remain well.

As a team, we deliver psycho-social interventions, advocacy, housing and welfare rights advice, peer support to adults as well as tailored support to young people (those under the age of 25).All we do is underpinned by listening and ensuring people feel heard – because we know that when people are listened to, they recover. The team works with colleagues within the mental health, primary care, inpatient and community sectors.

Role Purpose

As a Peer Support Worker, you will have lived or living experience of receiving mental health care or treatment in hospital. Using your expertise gained through this experience, you will support peers in their mental health recovery who are ready to leave hospital or are already in the community. Receiving support from your colleagues, you’ll develop and manage your casework practice and hold a small case load. 

You will walk alongside people to set achievable goals to ensure they get the support that they need, ultimately feeling connected to their communities – the right services and support network.

This role will work with a small but intensive caseload (max 12 cases for a full-time peer support worker) to focus on: 

• Repatriate: when people are sent to an out of area placement (OAPs) advocate to get them back to Greater Manchester so they can recover in their community. 

• Discharge: Supporting people when they’re ready to be discharged from inpatient mental health services to get back to the community and recover. 

• Prevent re-admissions: Work with people, post-discharge, to ensure they continue to get the support they need to make informed choices, stay well and ultimately thrive.

Main Duties and Responsibilities 

• To provide bespoke support to people that are ready to be discharged from inpatient psychiatric care to return to the community, building (with support) packages of care so they have a positive and sustained recovery. 

• To work alongside people post-discharge, in their community setting (e.g. home or supported accommodation) to ensure they get what they need to stay well. 

• To effectively manage a small caseload (about 7 people), being flexible to people’s needs and short-term changes in schedule. 

• To work in a person-centred, trauma informed, empowering and culturally appropriate way that enables people to feel listened to and heard.

• To help people engage with services and make sustainable links with their communities. 

• To use and develop your knowledge of community care, policies, legislation and services to get people the support they need to recover and stay well. 

• Identify when cases need escalation or onward referral and keep your line manager aware of case developments. 

• Independently visit people in inpatient and community settings in Manchester and across Greater Manchester, adhering to lone working and risk management procedures. 

• Occasionally visit people out of the Greater Manchester area who need to return to their community in Manchester (all travel expenses will be paid and non-commute travel time will be part of your working day). 

• Build relationships with colleagues within services (e.g. inpatient staff, social care, CMHTs etc) to enable improved collaborative working 

• Take part in multi-disciplinary meetings, representing the views of people, and their aspirations on returning to community settings. 

• Support the ongoing service development, drawing on your role as an ‘Expert by Experience’ and using what you hear from the people you support to contribute to the development of the services by reviewing and contributing to improvements.

Role Context

Recovery in Community is a successful partnership between Manchester Mind and Gaddum. We support people to manage and live with their mental health in their community.  Staff are employed by both organisations but work together collaboratively and as a team.

As the Recovery in Community Worker (Referral and Assessment) you will be supporting people in who have been referred into Greater Manchester Mental Health (GMMH) services but may not meet the threshold for secondary or specialist mental health services. The aim of this work is to ensure that people referred receive the right support at the right time.

You will work closely with the Referral and Assessment team at Greater Manchester Mental Health Trust (GMMH) to take referrals and ensure people get access to the mental health and social support they need.

Role Purpose

You will be building trusting and kind relationships with the understanding that they enable people to be listened to and heard and that this makes a difference. You will be part of a team providing effective person-centred support to people who come through the referral and assessment hub – many of whom often have tried many times to get the help they need.  We want to enable people to feel connected within their communities – to services, friends and family.

You will also be building relationships with NHS and statutory services. The structure of the service is brief intervention (working with people for a short period of time) with a focus on supporting people to connect to community services and resources effectively.

You will be well supported in this role. The service takes a team-based approach, and you will be joining a team of five and you will be supported directly by a Team Coordinator as well as being part of the wider Recovery in Community Service.

Main Duties and Responsibilities

· Take referrals from the GMMH (Greater Manchester Mental Health) Referral and Assessment Hub and build trusting relationships with people so that they receive the support they need to recover and stay well.

· Build relationships with other key professionals to ensure individuals achieve their goals (e.g. GPs, social care, community services etc).

· Support the development of a supportive and productive referral route into and out of the service, managing exit from the service safely.

· Support the development of this new service, responding professionally to changes as we improve and evolve our working practice collaboratively.

· Provide a trauma-informed service to people: listening, offering safety, choice and empowerment to the people you support so they can effectively engage with services and make sustainable links in with their communities.

· Identify and deliver appropriate interventions.

· Identify when people need referrals to secondary services or specialist mental health services and make appropriate referrals.

· Independently travel to visit people in a range of settings across in Manchester and across Greater Manchester, adhering to lone working and risk management procedures.

Richmond Borough Mind is a vibrant local charity that has been supporting and providing services for adults affected by mental health problems and their carers for over 60 years. 

We are excited to be looking for a Young Adults Peer Support Worker. You will have your own lived experience of mental ill health and be able to use this experience as well as your professional skills and knowledge, to support others.

With the expansion of our work in this area we aim to reduce waiting times, reduce the number of people going into crisis, address inequalities and support transitions from youth to adult services. We will be offering holistic and personalised care and support to service users and carers.

This role will carry caseload and deliver a range of support and person-centred interventions to meet these needs, which could include group support, one-to-one interventions and signposting where specialist advice is needed. Referrals could be young adults, careleavers, people currently under adult mental health services, or carers supporting people in the community.

We are looking for individuals who are passionate about motivating and coaching individuals with mental health issues, and able to support people on their journey to recovery.
