Job description

Are you committed to improving lives for vulnerable people?  

Community Connections Lewisham (CCL) is a preventative social prescribing project, delivered by Age UK Lewisham and Southwark, aiming to improve the health and wellbeing of vulnerable adults across the borough of Lewisham. We achieve this through working with both individuals who want connecting to services, and the community groups they access. We support people to find:

  • Local community groups and access to social opportunities
  • Help with staying active
  • Befriending and practical assistance (food shopping, odd jobs, etc.)
  • New opportunities for employment, training and learning
  • Volunteering opportunities and opportunities for participation in local projects

Crucially, our project relies on strong and healthy relationships with our Key Partners. These Partners are to whom we refer in order to support our Client base. We currently have over 20 Key Partners varying in size and scope - some are small, local charities; others are statutory bodies operating across the City.

Partnership Coordinators hold a key role in the effective delivery of the project, driving it forward through their work. A varied role, Coordinators support vulnerable people directly over the phone and face-to-face, while also being responsible for relationships with our Key Partners across the borough. This ensures a healthy mix of public and professional-facing duties.

We are looking for somebody who is comfortable talking to a wide range of professionals as well as members of the public by telephone, letter, email, and in person. You will be the point of contact for CCL which involves taking referrals by telephone and email, processing them in line with our systems and making onward referrals. The onward referrals we make will be to our Key Partners, the relationships with whom you will build and maintain.

This is a busy and rewarding role giving you the opportunity to make a real difference.  There will be the opportunity to attend events, forums and meetings to broaden your understanding of Health and Social Care and build experience working in the voluntary sector.  You will also be working closely with our other teams in both Lewisham and Southwark.
