Job description


North Notts Moving Forward is a floating support service for people with mental health needs, based at The Exchange in Newark.  We support people to live in the community with greater independence and improved quality of life.

You will have to help identify individual support needs, deliver support via individually tailored support plans to help the service users achieve their goals, and help review their progress and achievements. You will be expected to work effectively within a team and carry your own caseload.

You will be expected to have a good working knowledge of the needs of people struggling with mental health issues.  Experience of completing case files would be helpful, as well as a good working knowledge of benefits, housing law, debt support and any other issues related to the person’s ability to live independently.

You must have a current driving licence and access to a car.
